Netraumatický kompartment syndrom je závažný stav, který postihuje svalové prostory typicky u nemocných po ischemickém inzultu při akutní končetinové ischemii a může mít vážné následky pro postiženou končetinu a celkový zdravotní stav pacienta. Tento syndrom je charakterizován zvýšeným tlakem v uzavřeném intrafasciálním prostoru, což vede ke kompresi svalů, cév a nervů vedoucí k prohloubení ischemie a až nevratnému poškození těchto struktur. Včasná diagnóza a léčba jsou klíčové pro minimalizaci rizika komplikací a zachování funkce končetiny. V tomto článku se zaměříme na definici, patofyziologii a důležité klinické aspekty netraumatického kompartmentového syndromu.
Non-traumatic compartment syndrome is a serious condition that affects muscle compartments typically in patients after ischemic insult during acute limb ischemia and can have serious consequences for the affected limb and the patient's overall health. This syndrome is characterized by increased pressure in the intrafascial space, leading to compression of muscles, blood vessels and nerves leading to worsening ischemia and even irreversible damage to these structures. Early diagnosis and treatment are crucial to reduce the risk of complications and preserve limb function. In this article, we review the definition, pathophysiology and important clinical aspects of non-traumatic compartment syndrome.
Lékaři se v klinické praxi často setkávají s pacienty, u nichž byla zjištěna zvýšená hladina sérové kreatinkinázy (CK) bez objektivních projevů svalového onemocnění. Standardní laboratorní referenční rozmezí neodpovídá distribuci hodnot sérové CK v populaci, navíc fyzická aktivita může vést k výrazné elevaci CK, proto jsou abnormální výsledky běžně zjišťovány i u zcela zdravých osob. Primární myopatie jsou vzácné, u nemocných s přetrvávající elevací CK je vždy třeba nejprve vyloučit jiné příčiny včetně metabolických poruch, endokrinopatií a nežádoucích účinků léčiv. Ani svalová biopsie nemusí vést k určení diagnózy a měla by být prováděna jen v indikovaných případech. V posledních letech se při diagnostice subklinicky probíhajících myopatií uplatňují i moderní genetické metody označované jako next generation sequencing. Cílem tohoto textu je seznámit čtenáře s problematikou interpretace výsledků sérové CK, představit diferenciální diagnostiku zvýšených hodnot a poskytnout základní návod jak postupovat při pátrání po příčině.

Approach to a patient with elevated serum creatine kinase Physicians frequently encounter patients with elevated serum creatine kinase (CK) levels in clinical practice without objective signs of muscle disease. The standard laboratory reference range does not reflect serum CK distribution in the population. Moreover, physical activity can lead to a marked CK elevation; therefore, abnormal results are frequently reported in otherwise healthy subjects. Primary myopathies are rare; other causes, including metabolic disorders, endocrinopathies, and adverse drug reactions, should always be excluded first in patients with persistent CK elevation. Muscle biopsy does not always clarify a diagnosis and should only be performed in selected cases. In recent years, modern genetic methods, referred to as next-generation sequencing, have played a role in diagnosing subclinical myopathies. This paper aims to acquaint the reader with the issues involved in interpreting serum CK results, introduce the differential diagnosis of elevated values, and provide basic guidance on how to proceed in searching for a cause.

External load influences internal load in resistance training (RT). The purpose of the present study was to compare the total volume-load, perceptual and stress responses during three different RT protocols. Twelve resistance-trained men completed three different RT protocols with the back squat and bench press exercises: (1) power (POW) (5 sets of 6 repetitions at 50%1RM, 2-min of rest), (2) hypertrophy (HYP) (5 sets-to-failure at 75%1RM, 2-min of rest), and (3) strength (STR) (5 sets-to-failure at 90%1RM, 3-min of rest). Volume-load (kg × reps.), session rating of perceived exertion (sRPE), training impulse (TRIMP; reps. × sRPE), cortisol, immunoglobulin A (IgA), lactate, and creatine kinase (CK) were assessed before and/or after the sessions. HYP was the most demanding session in terms of volume-load (p < 0.001), TRIMP (p < 0.001), cortisol (p = 0.001), lactate (p < 0.001), and CK (p = 0.001). Despite POW exhibited a greater volume-load than STR (p = 0.016), the latter exhibiting a greater sRPE (p < 0.001), and a greater post-session CK (p = 0.05). However, the TRIMP of STR and POW were not statistically different (152 vs. 260 AU; p = 0.089). These specific responses could be meditated by the presence of muscular failure. When pooling all the sessions, significant correlations were revealed among external and internal stress markers (r = 0.35-0.80; p ≤ 0.05). The use of TRIMP could be recommended as a simple and valid monitoring tool which integrates into a single parameter the internal and the external loads of RT sessions.

Merrigan, JJ, Tufano, JJ, Fields, JB, Oliver, JM, and Jones, MT. Rest redistribution does not alter hormone responses in resistance-trained women. J Strength Cond Res 34(7): 1867-1874, 2020-The purpose was to examine acute effects of rest redistribution (RR) on perceptual, metabolic, and hormonal responses during back squats. Twelve resistance-trained women (training age 5 ± 2 years; one repetition maximum [1-RM] per body mass, 1.6 ± 0.2) performed traditional (TS, 4 sets of 10 repetitions with 120 seconds interset rest) and RR sets (4 sets of two 5 repetition clusters with 30-second intraset rest and 90-second interset rest) in counterbalanced order, separated by 72 hours. Both conditions were performed at 70% 1RM with 360 seconds of total rest. Ratings of perceived exertion (RPE) were taken after each set. Blood was sampled at baseline, after each set, and at 5, 15, 30, and 60 minutes, as well as 24 and 48 hours after training. Alpha level was p ≤ 0.05. The RPE progressively increased throughout both conditions (p = 0.002) with a greater overall mean for TS (5.81 ± 0.14) than RR (4.71 ± 0.14; p = 0.003). Lactate increased above baseline and remained elevated through 15 minutes post in both conditions (4.00 ± 0.76; p = 0.001), with greater lactate levels for TS (6.33 ± 0.47) than RR (4.71 ± 0.53; p < 0.001). Total testosterone was elevated after set 2 (0.125 ± 0.02; p = 0.011), but no other time point, while free testosterone remained unchanged. Growth hormone continually rose from baseline to set 3 and returned to baseline by 60 minutes post (20.58 ± 3.19). Cortisol and creatine kinase did not change over time. No condition × time interactions existed for any hormone (p > 0.05). Use of rest redistribution resulted in lower perceived effort and lactate responses. Yet, hormone responses during rest redistribution were no different from TS.

Biochemical and hematological examination of blood and individual assessment of the birds were performed in Lohman Brown laying hens at 45 weeks of age housed in different systems. The biochemical examination revealed higher (p < .01) corticosterone levels, creatine kinase, and aspartate aminotransferase activity and lower (p < .01) levels of lactate, triglycerides, albumin, calcium, and phosphorus in aviary hens compared to hens housed in furnished cages. Hematological examination of hens housed in aviaries revealed higher (p < .05) hematocrit, leukocytes, heterophils, eosinophils, basophils, monocytes, and H/L ratio. Furthermore, hens housed in aviaries had lower (p < .01) body weight than hens in furnished cages, they were worse feathered (p < .001), had more damaged combs (p < .05), and poorer physical condition (p < .01). In contrast, caged hens showed worse (p < .01) feather condition of the wings due to abrasion and claws due to overgrowth. The results have shown that the housing system has a significant impact on the internal environment and condition of birds and that housing in aviaries without taking into account the specifics of such housing may lead to significant stress and disturbance to the welfare of laying hens.

Diagnostický algoritmus svalových dystrofií se v uplynulé dekádě výrazně změnil, a to zejména díky rozvoji a zvýšení dostupnosti molekulárně genetických a zobrazovacích metod. Aktuálně, kromě podrobně odebrané anamnézy, detailního klinického vyšetření, biochemického a elektrofyziologického testování, přibylo vyšetření svalů magnetickou rezonancí a nové metody molekulárně genetického vyšetření, naopak svalová biopsie přestala být nezbytnou metodou v diagnostice hereditárních myopatií. Rutinní provádění MRI u pacientů se svalovými dystrofiemi umožnilo odhalení a popsání vzorců svalového postižení (pattern of recognition) charakteristických pro určité klinické jednotky. Molekulárně genetická vyšetření pak, jako jediná, umožňují stanovení definitivní diagnózy na základě detekce kauzální mutace. Pro lepší orientaci v běžné ambulantní praxi popisuje článek hlavní kroky vedoucí k odhalení jednotlivých typů svalových dystrofií s ohledem na úroveň dnešních znalostí a zkušeností.
The diagnostics algorithm of muscular dystrophies has changed significantly over the past decade, mainly due to the development and increase of availability of molecular genetics and imaging methods. The golden standard of detailed medical history, attentive clinical examination, biochemical and electrophysiological testing now includes also magnetic resonance imaging and targeted or more extensive molecular genetic examinations, while muscle biopsy ceased to be the first choice method in the diagnostic process of hereditary myopathies. Routine MRI performance in patients with muscle. dystrophies allowed the detection and description of patterns of recognition characteristic for certain clinical units. Molecular genetic examinations as the only one allow definitive diagnosis to be determined by causal mutation detection. For better orientation in common outpatient practice, the article describes the crucial steps leading to the discovery of individual types of muscular dystrophy with respect to the level of today's knowledge and experience.

Poor neuromuscular control and fatigue have been proposed as a risk factor for non-contact injuries especially around peak height velocity (PHV). This study explored the effects of competitive soccer match-play on neuromuscular performance and muscle damage in male youth soccer players. 24 youth players aged 13-16y were split into a PHV group (-0.5 to 0.5y) and post PHV group (1.0-2.5y) based on maturity off-set. Leg stiffness, reactive strength index (RSI), muscle activation, creatine kinase (CK), and muscle soreness were determined pre and post a competitive soccer match. Paired t-tests were used to explore differences pre and post competitive match play and independent sample t-tests for between groups differences for all outcome measures. There was no significant fatigue-related change in absolute and relative leg stiffness or muscle activation in both groups, except for the gastrocnemius in the post PHV group. RSI, CK and perceived muscle soreness were significantly different after soccer match-play in both groups with small to large effects observed (ES:0.41-2.82). There were no significant differences between the groups pre match-play except for absolute and relative leg stiffness (P < 0.001; ES = 1.16 and 0.63 respectively). No significant differences were observed in the fatigue related responses to competitive match play between groups except for perceived muscle soreness. The influence of competitive match-play on neuromuscular function and muscle damage is similar in male youth around the time of PHV and those post-PHV indicating that other factors must contribute to the heightened injury risk around PHV.

OBJECTIVES: Increasing muscular hypertrophy is one of the main reasons for participating in a resistance training program, where different movement task such as eccentric cadences may serve as a potent hypertrophic stimulus and improve movement stability. Aim of this study was to investigate the physiological responses between slow 6/0/2/0 (SLOW) and moderate 2/0/2/0 (REG) eccentric cadences during five sets of bench press to failure using 70% 1 repetition maximum (1RM). MATERIALS AND METHODS: Blood samples from sixteen men (21-29y, 85.9±7.7kg, 130±17.5kg bench press 1RM) with at least five years of resistance training experience were taken before, immediately after, 30 min after, and 60 min after both protocols in a randomized cross over study design. RESULTS: ANOVA showed that more repetitions were performed during each set in REG and for the entire REG protocol (p<0.001), but total time under tension was greater during SLOW in each set and for the entire protocol (p<0.001). The post-exercise levels of lactate (p=0.02), creatine kinase (p=0.04), and testosterone (p=0.01) were greater after SLOW. Post-exercise cortisol levels decreased in both protocols (p<0.001), but these decreases were not significantly different between protocols. CONCLUSIONS: Therefore, intentionally slow eccentric speeds and increased eccentric time under tension seem to be effective for increasing acute hormonal responses after exercise. As such, although a SLOW tempo may decrease the amount of total work (i.e. fewer repetitions with the same load), the increased time under tension seems to drive hormonal responses and neurological response, which may play a large role in stimulating muscle growth, coordination and movement stability.

Dilatační a hypertrofické kardiomyopatie, poruchy srdečního rytmu nebo kombinace obojího mohou patřit do obrazu některých dědičných a/nebo získaných chorob kosterního svalstva. Na tyto širší souvislosti může upozornit jednoduché, široce dostupné vyšetření sérové kreatininkinázy. Pacienti se suspekcí na myopatii s kardiomypatií by měli být odesláni k došetření neurologům. Specializovaným laboratorním vyšetřením séra, elektromyograficky, svalovou biopsií a genetickým vyšetřením jsme schopni detekovat získané či dědičné důvody postižení kosterního a srdečního svalsta. Rozpoznání příčiny kardiálního selhání je důležité ke stanovení komplexního léčebného postupu u nemocných i podchycení rizik život ohrožující kardiomyopatie u jejich potomků. Uvádíme osm krátkých kazuistik myopatie s kardiomyopatií. Cílem práce je upozornit na průsečík našich odborností, seznámit kardiologickou odbornou veřejnost s jednoduchým návodem, jak odhalit myopatii a možnostmi neurologů stran jejího došetření.
Dilated and hypertrophic cardiomyopathies, arrhythmias, and/or a combination of these conditions, may be associated withfamilial and/or acquired skeletal muscle diseases. Widely available serum creatine kinase assays can readily indicate the presenceof skeletal muscle damage. Patients with diagnosed cardiomyopathy and suspected skeletal myopathy should be evaluatedby a neurologist capable to of detecting inherited or acquired comorbidities of skeletal and cardiac muscle damage by usingspecialized serum laboratory tests, electromyography, skeletal muscle biopsy, and genetic testing. This strategy will aid inestablishing a comprehensive treatment regimen, and contribute to defining the risk of life-threatening cardiomyopathy andheart failure in the offspring. We present a study of eight patients with skeletal myopathy accompanied by cardiomyopathy.The aim of this report is to draw attention to the multidisciplinary implications of skeletal myopathy and cardiomyopathy aswell as to alert cardiologists, neurologists, and other health care professional to the potential for comorbidity of skeletal andcardiac muscle disease.
