Cíl: Prezentace vlastních zkušeností s kvantitativním měřením krevního průtoku (flowmetrie) magistrálních tepen při operacích mozkových aneuryzmat. Soubor a metodika: Intraoperační flowmetrie umožňuje intraoperační kvantitativní stanovení průtoku v měřené cévě v ml/min. V období 1/2011–5/2013 bylo kvantitativní měření krevního průtoku magistrálních tepen při operacích mozkových aneuryzmat provedeno u 23 pacientů. Průměrný věk činil 52,1 let (30–73). V 19 případech se jednalo o incidentální aneuryzmata, ve čtyřech případech bylo přítomno subarachnoidální krvácení (2krát Hunt-Hess I, 2krát Hunt-Hess II). Lokalizace aneuryzmat: aneuryzma střední mozkové tepny 16krát, přední komunikující tepny 4krát, zadní komunikující tepny 2krát a a. pericallosa jednou. Velikost vaku aneuryzmat: malá (do 7 mm) 10, střední (8–14 mm) devět, velká (15–24 mm) tři a gigantické (nad 25 mm) jedno. Výsledky: Třicetidenní pooperační výsledky: v 21 případech normální nález a ve dvou případech mírný deficit. V souboru nebyla zaznamenána pooperační ischemie. Patologické intraoperační hodnoty krevního průtoku se vyskytly u čtyř pacientů. Ve dvou případech byl příčinou vazospazmus. Normalizace hodnot průtoku nastala po podání papaverinu. Ve dvou případech (8,7 %) byla provedena korekce polohy klipu s následnou normalizací průtoku. Závěr: Kvantitativní měření krevního průtoku magistrálních tepen přispívá ke zvýšení perioperační bezpečnosti chirurgie mozkových aneuryzmat. Úloha flowmetrie při detekci stenózy/okluze magistrálních tepen je nezastupitelná.

Aim: To present own experience with quantitative flow measurement (flowmetry) of parent and branching arteries during surgical treatment of cerebral aneurysms. Material and methods: Intraoperative flowmetry enables quantitative blood flow measurement in ml/min on the basis of integration of the ultrasound beam transit time difference. Between 1/2011 and 5/2013, quantitative blood flow measurement of parent and branching arteries was performed in 23 patients during cerebral aneurysm surgery. The mean age was 52.1 years (30–73). Incidental aneurysms were present in 19 cases; four patients had subarachnoid hemorrhage (Hunt-Hess I in two and Hunt-Hess II in two). Location: middle cerebral artery aneurysm – 16 patients, anterior communication artery aneurysms – four patients, posterior communication artery aneurysm – two patients, and distal anterior cerebral artery aneurysm – one patient. Size of the aneurysm: small (≤ 7 mm) in 10 patients, middle (8–14 mm) in nine patients, large (15–24 mm) in three patients and giant (≥ 25 mm) in one patient. Results: Thirty-day postoperative results: good recovery in 21 cases and moderate disability in two cases. No postoperative ischemia was recorded in the group of patients. A significant perioperative blood flow decline was recorded in four patients; this was due to vasospasm in two and the flow normalized after papaverin administration. Clip correction was necessary in two patients (8.7%) followed by normalization of the flow. Conclusions: Quantitative blood flow measurement contributes to improved perioperative safety in cerebral aneurysms surgery. Role of flowmetry is irreplaceable in detection of parent and45 branching artery stenosis/occlusion. Úvod: Vzhledem ke stálému prodlužování délky života se analýza výsledků chirurgických operací u nemocných ve vyšším věku dostává do stále většího zájmu cévních chirurgů. Cílem naší studie bylo zhodnotit výsledky operací aneurysmat břišní aorty (AAA) u pacientů vyššího věku (75 let a více) a srovnat je s mladší populací. Metoda: Celkem bylo analyzováno 171 pacientů, kteří podstoupili operaci AAA v jednom centru v letech 1999–2008. Mezi skupinami nemocných (starší vs. mladší, s rozšířeným hemodynamickým sledováním [EHM], bez EHM) byla srovnávána četnost perioperačních a pooperačních komplikací, 30denní mortalita, délka pobytu na JIP, celková délka hospitalizace. Pro statistickou analýzu byl použit Kruskalův-Wallisův test a χ2 test. Výsledky: Nebyl zjištěn statisticky významný rozdíl ve výskytu onemocnění v anamnéze pacientů před operací, pouze ICHS a chronická renální insuficience (CHRI) se vyskytovaly častěji u starších nemocných. Frekvence perioperačních a pooperačních komplikací: kardiálních, respiračních, CNS, chirurgických včetně výskytu sepse a závažného pooperačního krvácení se nelišily u jednotlivých věkových skupin nemocných, stejně jako nebyl zjištěn statisticky významný rozdíl v mortalitě a délce hospitalizace. Statisticky významný rozdíl byl pouze ve frekvenci pooperačního renálního poškození a délce pobytu na JIP. Nemocní starší 75 let bez EHM byli na JIP signifikantně déle (pět dnů vs. sedm dnů, p < 0,001) a rovněž měli signifikantě častěji zhoršené pooperační renální funkce (p = 0,002). Závěr: Potvrdili jsme akceptovatelné výsledky operací AAA u pacientů ve věku 75 let a více ve srovnání s mladší populací. Elektivní chirurgie pro aneurysma břišní aorty může být bezpečně provedena i u nemocných ve vyšším věku, avšak ostatní možnosti léčby by měly být rovněž zváženy.

Introduction: Given the steady increase in life expectancy, an analysis of surgical outcomes in the aging population is of significant interest to vascular surgeons. The aim of our study was to assess the outcomes of abdominal aortic aneurysm repair in the elderly aged 75 plus and compare their results with people operated on at a younger age. Methods: 171 patients who underwent open AAA repair in one centre in 1999–2008 were analyzed. The perioperative and postoperative complications, 30-day mortality, the length of ICU and hospital stay were compared, and the Kruskal-Wallis and the Chi-square tests were used for statistical analysis. Results: There were no statistically significant differences in gender and patients’ preoperative comorbidities except for ICHS and CHRI which were more frequent in the elderly 75 plus. The frequency of serious perioperative haemodynamic complications and postoperative (cardiac, respiratory, cerebral, bleeding, sepsis, surgical) complications, mortality rate and the length of hospital stay did not significantly differ in the elderly 75 plus. A statistically significant difference was found only in the frequency of postoperative renal damage and the length of ICU stay. The elderly 75 plus without enhanced haemodynamic monitoring had significantly longer ICU stay (5 days vs. 7 days, p<0.001) and developed renal damage significantly frequently (p=0.002). Conclusion: We confirmed acceptable outcomes of patients aged 75 plus who underwent open AAA repair compared with younger population. Elective surgery for AAA can be safely performed in the elderly patients, however, other possibilities of treatment should also be considered.

Mnohé chirurgické výkony v oblasti hlavy a krku sú rizikové z hľadiska poškodenia hlavových a periférnych nervov. Príčinou poškodenia sú najčastejšie mechanická trauma a ischémia. Po operácii sa to prejaví bolesťou, funkčnými poruchami a v prípade tvárového nervu až mutilujúcimi kozmetickými zmenami v inervačnej oblasti nervu. Elektrofyziologické merania motorických nervov počas operácie podávajú informácie o lokalizácii a funkčnom stave nervu a pomáhajú minimalizovať rozsah poškodenia. V práci prezentujeme retrospektívnu analýzu súboru pacientov operovaných s použitím peroperačného monitoringu hlavových nervov. 122 pacientov operovaných pre tumor v oblasti pontocerebelárneho uhla (PCU) translabyrintným (TL) prístupom, 22 pacientov operovaných retrosigmoidným (RS) prístupom, 20 pacientov operovaných pre vrodené vývojové chyby (VVCH) vonkajšieho a stredného ucha, 5 pacientov operovaných pre glomus tumor infratemporálnym prístupom A podľa Fischa s premiestnením tvárového nervu (n. VII), 144 pacientov operovaných pre tumor príušnej slinnej žľazy, 12 pacientov operovaných pre malígny tumor štítnej žľazy alebo reoperáciu. Signifikantný bol vzťah Bergesovho pomeru ku veľkosti tumoru PCU do 2 cm (p< 0,001), ako aj závislosť funkcie n. VII v House-Brackmannovej klasifikácii (HB) 10. pooperačný deň pri použitej stimulačnej intenzite 0,05 mA, oproti skupine pacientov s intenzitou nad 0,1 mA ( p< 0,001). Signifikantná bola aj závislosť funkcie n. VII v HB klasifikácii po 1 roku od operácie v skupine pacientov s použitou stimulačnou intenzitou 0,05 mA oproti skupine pacientov s použitou intenzitou nad 0,1 mA (p< 0,001).

Most of surgical procedures in head and neck region are dangerous because of cranial and peripheral nerve damage possibility. Mechanical trauma and ischemia are responsible for damage of nerves. Pain, functional disorders even cosmetic alteration of face can be observed after surgery. Intraoperative nerve monitoring presents information about position and functional status of the nerve in operating field and allow to minimize nerve damage during surgery. We present retrospective analysis and our findings in group of patients underwent surgery in head and neck region. Surgery was conducted under intraoperative monitoring of cranial nerves. Group of 122 patient underwent surgery in region of cerebellopontine angle (CPA) via translabyrinthine approach, 22 patients underwent surgery via retrosigmoid approach, 20 patients underwent surgery because of congenital malformations of external or middle ear, 5 patients underwent surgery via Fisch infratemporal approach A with transposition of facial nerve because of glomus tumor. 144 patients underwent surgery because of parotid gland tumor and 12 patients underwent surgery because of malignant thyroid gland tumor or revision surgery in this region. Relation of CPA tumor with amount till 2 cm to Berges formula average was confirmed as most significant (p< 0,001). Relation of early postoperative n.VII function with stimulation threshold 0,05 mA to early postoperative n. VII function with threshold over 0,1 mA was confirmed as significant (p< 0,001) and also relation of late postoperative n.VII function with stimulation threshold 0,05 mA to late postoperative n. VII function with threshold over 0,1 mA was confirmed as significant (p< 0,001).

V práci autoři prezentují první zkušenosti s využitím peroperačního ultrazvukového vyšetření při dekompresi velkého týlního otvoru u Chiariho malformací typu I. Zaměřují se na peroperační diagnostiku blokády s kontrolou zprůchodnění likvorových cest v průběhu dekomprese v malformované cerebrospinální junkci. Na malém souboru sedmi pacientů operovaných v letech 2004–2009 na dvou pracovištích byla provedena peroperačně diagnostika blokády zároveň s kontrolou obnovení průtoku likvoru pomocí kombinace ultrazvukového vyšetření v B obraze a dopplerovského vyšetření. Pooperační výsledky potvrdily, že pomocí ultrazvukového vyšetření byla správně detekována blokáda a následně obnoven průtok likvoru během operace. Využití ultrazvukového vyšetření napomohlo rovněž k adekvátnímu chirurgickému ošetření postižené oblasti, a to prostou dekompresí či dekompresí s plastikou pleny u šesti pacientů. Zdá se, že duplexní ultrazvukové vyšetření kombinující zobrazení tkání v B obraze a detekci toku v dopplerovském modu by mohlo být vhodnou alternativou diagnostického vyšetření u tohoto typu onemocnění.

First experiences with the use of intra-operative ultrasound examination in the course of foramen magnum decompression to treat type 1 Chiari malformation are presented. The authors focus on the intra-operative diagnostics of the obstruction together with control of the patency of cerebrospinal fluid (CSF) pathways at the malformed cerebrospinal junction. A small group of 7 patients operated upon in the years 2004 – 2009 at two neurosurgical departments underwent intra-operative ultrasound diagnostics by examination in B-mode and dopplerometric measurement. Postoperative results demonstrated that ultrasound examination properly detected an obstruction and subsequently monitored restoration of CSF flow in the course of operation. The use of ultrasound examination contributed to satisfactory surgical treatment in the sense of simple bone decompression or bone decompression with duraplasty in 6 patients. Duplex ultrasound imaging in combination with B mode imaging and dopplerometric measurement of CSF flow may be considered a suitable approach to diagnostic examination in this disease.

Naším cílem bylo určit ideální množství heparinu na kg hmotnosti pacienta a množství protaminu potřebné k následné neutralizaci dle hodnot aktivovaného koagulačního času (ACT). Soubor a metodika: Do prospektivní studie bylo zařazeno 80 pacientů (20 žen, 44 asymptomatických stenóz, průměrný věk 65,8; rozpětí 39–82 let, bilaterální stenóza u čtyř pacientů), kteří v období od dubna 2008 do května 2009 podstoupili 84 karotických endarterektomií (CEA). Pacienty jsme rozdělili do tří skupin podle podané dávky heparinu (1; 1,5 a 2 mg heparinu na kg hmotnosti). Výsledky: Medián ACT po aplikaci heparinu byl v první skupině 268,0; ve druhé skupině 334,0 a ve třetí skupině 375,0 (p < 0,01). Medián ACT po podání protaminu byl v první skupině 130, ve druhé 137 a ve třetí 128 (p = 0,79). Pooperačně se vyskytlo celkem sedm komplikací, z nichž byly dva hematomy v místě rány s nutností chirurgické revize (2,4 %), tři neurologické komplikace (3,6 %) a dvě kardiologické komplikace (2,4 %). Při ambulantní kontrole za tři měsíce se stav všech pacientů upravil ad integrum, permanentní morbidita tedy činí 0 %. Závěr: K dosažení adekvátní antikoagulace postačuje dávka 1 mg heparinu na kg váhy, tato dávka minimalizuje riziko vzniku pooperačního cervikálního hematomu a nezvyšuje riziko vzniku perioperační ischemické cévní mozkové příhody (iCMP).

The aim of this study was to determine the ideal amount of heparin per kilogram of patient body weight and the amount of protamine necessary for its subsequent neutralization with relation to initial heparin dose and with respect to activated coagulation time (ACT). Patients and methods: The prospective study group consisted of 80 patients (20 female, 60 male) 44 of them treated for asymptomatic stenosis, mean age 65.8, range 39–82 years, bilateral stenosis in 4 of them, upon whom a total of 84 carotid endarterectomies had been performed between April 2008 to May 2009. Patients were divided into three groups, according to the amount of heparin administered (1, 1.5 and 2 mg of heparin per kilogram). Results: In first group, median ACT after heparin administration was 268.0 seconds; in second 334.0 and in the third group 375.0 (p <0.01). Median ACT after protamine administration in the first group was 130 seconds, in the second 137 seconds and in the third group 128 seconds (p = 0.79). A total of 7 postoperative complications occurred, including 2 wound haematomas requiring surgical revision (2.4%), 3 neurological complications (3.6%) and 2 cardiological complications (2.4%). After three months all patients had recovered ad integrum at ambulatory check-up; permanent morbidity is thus 0%. Conclusion: One milligram of heparin per kilogram is a sufficient dose to achieve adequate anticoagulation, minimizing the risk of postoperative haematoma without increasing the risk of peri-operative ischemic stroke.

Cíl studie: Získání údajů o způsobu použití svalových relaxancií (NMBA) během celkové anestezie v České republice. Typ studie: Prevalenční jednodenní dotazníková studie. Typ pracoviště: Pracoviště poskytující anesteziologickou péči, která se zaregistrovala před termínem studie. Materiál a metoda: Projekt je součástí studie Czech Anesthesia Day 2010. Do ní byl zahrnut každý pacient na registrovaném pracovišti, kde byla zahájena anesteziologická péče 1. 6. 2010 mezi 7:00 a 24:00 h. Údaje o každém pacientovi byly vkládány do internetového on-line formuláře. Sledované ukazatele zahrnovaly parametry se vztahem ke svalové relaxaci (použití NMBA, antagonizace bloku, monitorování relaxace), základní demografické údaje (věk) a data o podané anestezii (typ výkonu, místo podání anestezie, způsob zajištění dýchacích cest). Výsledky: 2252 nemocných ve sledovaném období podstoupilo celkovou anestezii. Relaxovaných bylo 55,9 % nemocných. K tracheální intubaci bylo u plánovaných výkonů podáno nejčastěji nedepolarizující NMBA (55,7 %), u neplánovaných suxamethonium (49,4 %). K relaxaci bylo nejčastěji použito atrakurium (43,1 %), suxamethonium (31,2 %) a rokuronium (24,4 %), u malých dětí mivakurium (55,3 %). V ambulantním provozu byla relaxována 3 % nemocných, za hospitalizace 39,9 % (p < 0,0001). K reverzi bloku byl použit neostigmin (24,1 % nemocných), jednou byl podán sugammadex. Hloubka bloku byla monitorována u 5,1 % nemocných. Monitorovaní nemocní dostali častěji neostigmin. Svalové relaxans bylo podáno více než polovině nemocných, nejčastěji atrakurium, suxamethonium a rokuronium. Reverze neostigminem byla provedena u čtvrtiny nemocných. Monitorování relaxace je v ČR nedostatečné (5 % relaxovaných).

Objective: To obtain data about the use of neuromuscular blocking agents during general anaesthesia in the Czech Republic. Design: One-day, prevalence, questionnaire survey. Setting: All anaesthetic care providers that registered before the date of the study. Materials and methods. This survey was part of the Czech Anaesthesia Day 2010 study. All the patients whose anaesthetic care was provided by the registered anaesthetic departments and was commenced on June 1, 2010, from 7:00 to 24:00 were enrolled. Data of each patient were entered into a web-based questionnaire and information relevant to neuromuscular blockade was collected. Results: Total 2,252 patients were given general anaesthesia and neuromuscular blockade was used in 55.9% cases. Tracheal intubation for elective surgery was most often facilitated by a non-depolarizing agent (55.7%), and in emergency cases by suxamethonium (49.4%). Atracurium (43.1%), suxamethonium (31.2%) and rocuronium (24.4%) were the most frequently used agents. During day-case surgery, a neuromuscular blocking agent was used in 3% cases compared to 39.9% for in-patient surgery (p < 0.0001). To reverse the block, neostigmine was administered in 24.1% of patients and sugammadex was used in one patient. The degree of the neuromuscular block was monitored in 5.1%. Patients with neuromuscular monitoring received neostigmine more often than those without monitoring. Conclusion: A neuromuscular blocking agent was used during the maintenance of more than 50% of the general anaesthesia cases; atracurium, suxamethonium and rocuronium were the most frequently used drugs. A reversal agent (neostigmine) was given to 24% patients. Monitoring of the block was rare (5%).

Prvým cieľom práce bola experimentálna aplikácia krátkodobej inhalačnej anestézie xenónom a zvládnutie techniky a technológie jej podávania. Druhým cieľom bolo sledovanie objektívnych fyziologických a laboratórnych parametrov, ako aj subjektívnych pocitov experimentálnych subjektov – anestéziológov, ktorí sa podrobili anestézii. Autori v práci opisujú prvé experimentálne podanie xenónovej inhalačnej anestézie na dobrovoľníkoch. Pracovisko: Oddelenie anestéziológie a intenzívnej medicíny. Dizajn: Klinický experiment (Súhlas etickej komisie č. 21082009). Materiál a metodika: Na skupine 5 dobrovoľníkov sme sledovali účinky xenónovej anestézie, jej objektívnych účinkov na fyziologické premenné, laboratórne výsledky a subjektívne prežívanie anestézie do štádia, kedy dôjde k strate vedomia. Dva experimentálne subjekty voviedli do prvých dvoch štádií hĺbky anestézie, ďalšie 3 subjekty do 3–4. štádia xenónovej anestézie. Pacienti boli štandardne monitorovaní a zabezpečení. Výsledky:V parametroch laboratórnych biochemických a hematologických vyšetrení, porovnávali výsledky odberov pred anestéziou a cca 15 min po anestézii. Rozdiely medzi výsledkami pred anestéziou a po anestézii nezistili. Výsledky boli v tolerancii fyziologických hodnôt. Počas anestézie boli sledované parametre obehu (TK a P) stabilné a nedošlo k významným zmenám pulzu a TK. Ventilačné parametre sa v úvode do anestézie mierne menili, dýchanie sa spomalilo a prehĺbilo, ale po nástupe III a IV štádia anestézie „spánku“ sa parametre prakticky vrátili na pôvodné hodnoty. Monitorovaná SpO2 bola stabilná, okolo 97–100 %. ETCO2 sa pohybovalo od 5,2 do 4,6 %. Autori hodnotia subjektívne pocity anestézovaných subjektov, a to v momente dosiahnutia relatívne stabilnej koncentrácie xenónu v dýchacom okruhu pri 10, 20, 30, 40 a 50%. Záver: Autori konštatujú, že pre získanie určitej skúsenosti s aplikáciou xenónu, ako aj technologických zručností bol tento experiment veľkým prínosom. Z pohľadu experimentálnych osôb – anestéziológov, to bola nenahraditeľná skúsenosť. Autori predpokladajú skoré zavedenie xenónu do anestéziologickej praxe v indikovaných prípadoch na vlastnom oddelení.

Objective:We describe the first experimental administration of xenon inhalational anaesthesia in volunteers. The primary aim was administering a short inhalational xenon anaesthesia and learning the technique and technology of its administration. The secondary aim was observing objective physiological and laboratory parameters and the subjective feelings of the subjects who had undergone the anaesthesia. Setting: Department of Anaesthesia and Intensive Medicine. Design: Clinical experiment (Approval No. 21082009 of the Ethical Committee). Materials and methods:The objective effects of xenon anaesthesia on the physiological variables and laboratory tests and its subjective perception until the loss of consciousness by the subjects were tested in a group of 5 volunteers. Two subjects reached the first two depth levels of anaesthesia and the remaining subjects reached levels 3 and 4 of xenon anaesthesia. All the subjects were monitored and managed in a standard fashion. Results: The laboratory tests were performed prior to and 15 minutes after the anaesthesia. There were no significant differences found in the results before and after the anaesthesia. No result exceeded the normal range. Blood pressure and heart rate were stable, without significant changes, during the course of anaesthesia. Ventilatory parameters showed minor changes, the heart rate decreased and breathing became temporarily deeper but after reaching anaesthesia depth-levels 3 and 4, all the parameters became normal again. SpO2 was stable, oscillating between 97 and 100%, ETCO2 reached 4.6–5.2 kPa. The authors evaluated the subjective perception by the anaesthetized persons at the moments when xenon concentration in the airways reached a relatively stable concentration at 10, 20, 30, 40 and 50%. Conclusion: This experiment is one of the first ones in the field of gaining technical skills and experience in performing xenon anaesthesia. For the volunteers, all of whom are anaesthetists, this was a unique personal experience. The authors expect early implementation of xenon anaesthesia into their clinical anae - sthetic practice in indicated cases.

Cíl práce: Představit koncept péče o pacienty s akutní disekcí typu A na Kardiochirurgické klinice Univerzitní nemocnice v Essenu. Metodika: Koncept je postaven na čtyřech základních principech: minimalizaci času od stanovení diagnózy do vlastní operace, optimální volbě arteriální kanylace na základě předoperačního vyšetření přímo na operačním stole hybridního sálu, odstranění těžké viscerální malperfuze předoperační fenestrací či stentingem v oblasti břišní aorty a využití perioperační angioskopie pro vyšetření aortálního oblouku a descendentní aorty pro stanovení nutného rozsahu vlastní operace. Výsledky: Od ledna 2004 do prosince 2008 bylo v Essenu operováno celkem 71 pacientů s akutní aortální disekcí průměrného věku 61 ± 13 let. V souboru převládali muži 44 (62 %) a pacienti s disekcí DeBakey typu I (59 %). Třicet šest hemodynamicky stabilních pacientů podstoupilo předoperační angiografii (51 %), u zbylých (pacienti v kardiogenním šoku či těžce nestabilní) byl proveden emergentní výkon bez předchozí angiografie. Sedm pacientů (10 %) podstoupilo před operací endovaskulární intervenční výkon (fenestraci či stenting). Během hypotermické cirkulační zástavy byla provedena perioperační angioskopie, na jejímž základě byl indikován rozsah operačního výkonu v oblasti oblouku a descendentní aorty. U všech pacientů byl proveden výkon na ascendentní aortě, u 67 na aortálním oblouku a u 30 na descendentní aortě. Za hospitalizace zemřelo celkem devět pacientů (13 %) a u sedmi pacientů (10 %) došlo v pooperačním období ke vzniku CMP, pět (7 %) bylo revidováno pro krvácení. Závěry: Essenský algoritmus péče o pacienty s akutní disekcí typu A zkracuje významně dobu od stanovení diagnózy do provedení operace, minimalizuje riziko dlouhodobé peroperační malperfuze a stanovuje jasná pravidla pro racionální užití kombinované protézy či provedení extenzivního výkonu v oblasti aortálního oblouku.

Aim of study: To present the concept of management of patients with acute aortic dissection, developed at the Department of Thoracic and Cardiovascular Surgery of University Hospital Essen, Germany. Methods: The concept is based on four main principles, i.e., minimization of time from diagnosis to actual surgery, optimal selection of arterial cannulation based on preoperative assessment performed right on the operating table of a hybrid operating room, termination of severe visceral malperfusion using fenestration or stenting in the abdominal aorta region, and use of intraoperative angioscopy to examine the aortic arch and descending aorta to determine the necessary extent of actual surgery. Results: A total of 71 patients with acute aortic dissection, with a mean age of 61 ± 13 years, were operated on from January 2004 through December 2008 in Essen, Germany. Patients were predominantly men (44; 62%) and those with DeBakey Type I dissection (59%). Thirty-six (51%) hemodynamically stable patients had preoperative angiography while the remaining (those with cardiogenic shock or severe hemodynamic instability) had emergency surgery without previous angiography. An endovascular intervention procedure (fenestration or stenting) was performed in seven patients (10%) preoperatively. Intraoperative angioscopy, undertaken during hypothermic circulatory arrest, served to determine the extent of surgery in the area of the aortic arch and descending aorta. The procedure involved the ascending aorta in all patients, aortic arch in 67, and descending aorta in 30 patients. There were nine in-hospital deaths (13%) while seven patients (10%) developed stroke, and fi ve (7%) required surgical revision for bleeding. Conclusions: The Essen algorithm of care of patients with acute aortic dissection shortens the time from diagnosis to surgery, minimizes the risk of long-term intraoperative malperfusion, and sets clear rules for the rational use of a combined prosthesis or an extensive procedure in aortic arch and descending aorta.
