ER stress has emerged as a signaling platform underlying the pathogenesis of various kidney diseases. Thus, there is an urgent need to develop ER stress biomarkers in the incipient stages of ER stress-mediated kidney disease, when a kidney biopsy is not yet clinically indicated, for early therapeutic intervention. Cysteine-rich with EGF-like domains 2 (CRELD2) is a newly identified protein that is induced and secreted under ER stress. For the first time to our knowledge, we demonstrate that CRELD2 can serve as a sensitive urinary biomarker for detecting ER stress in podocytes or renal tubular cells in murine models of podocyte ER stress-induced nephrotic syndrome and tunicamycin- or ischemia-reperfusion-induced acute kidney injury (AKI), respectively. Most importantly, urinary CRELD2 elevation occurs in patients with autosomal dominant tubulointerstitial kidney disease caused by UMOD mutations, a prototypical tubular ER stress disease. In addition, in pediatric patients undergoing cardiac surgery, detectable urine levels of CRELD2 within postoperative 6 hours strongly associate with severe AKI after surgery. In conclusion, our study has identified CRELD2 as a potentially novel urinary ER stress biomarker with potential utility in early diagnosis, risk stratification, treatment response monitoring, and directing of ER-targeted therapies in selected patient subgroups in the emerging era of precision nephrology.

BACKGROUND: Recent research has produced an explosion of experimental data on the complex neurobiological mechanisms of developmental disorders including autism. Animal models are one approach to studying the phenotypic features and molecular basis of autism. In this review, we describe progress in understanding synaptogenesis and alterations to this process with special emphasis on the cell adhesion molecules and scaffolding proteins implicated in autism. Genetic mouse model experiments are discussed in relation to alterations to selected synaptic proteins and consequent behavioral deficits measured in animal experiments. METHODS: Pubmed databases were used to search for original and review articles on animal and human clinical studies on autism. RESULTS: The cell adhesion molecules, neurexin, neurolignin and the Shank family of proteins are important molecular targets associated with autism. CONCLUSION: The heterogeneity of the autism spectrum of disorders limits interpretation of information acquired from any single animal model or animal test. We showed synapse-specific/ model-specific defects associated with a given genotype in these models. Characterization of mouse models with genetic variations may help study the mechanisms of autism in humans. However, it will be necessary to apply new analytic paradigms in using genetically modified mice for understanding autism etiology in humans. Further studies are needed to create animal models with mutations that match the molecular and neural bases of autism.

Ti implants have become an important instrument in handling various problems associated with the loss of fle¬xibility of a joint system. Having replaced a damaged joint with an endoprosthesis reduces pain, restores flexibility and improves the patient's quality of life. The implant-bone integration may be a complicated and lengthy process. Therefore, the state of the material surface is of vital importance. Ti can be treated electrochemically to produce a tubular nanostructure, which can later be modified to achieve bioactivity. Moreover, the large adsorption surface enables anchoring of biologically active substances and pharmaceuticals. The nanostructure and its further modifications could facilitate and strengthen the implant-bone integration, and thus shorten the healing process. The article describes the preparation of nanostructures and summarizes the knowledge of Ti surface treatment.

Periostin je matricelulární protein o m. h. 90 000, který byl poprvé prokázán v myších osteoblastech. Záhy byla doložena jeho přítomnosti u člověka. Svůj název získal díky vysoké expresi v periostu. Do povědomí pneumologů a alergologů se dostal především díky pracím prokazujícím závislost jeho produkce na stimulaci interleukiny 4 (IL-4) a 13 (IL-13) a transformujícím růstovým faktorem β (TGFβ). To otevřelo bránu k jeho využití jako biologickému znaku imunopatologických stavů spojených s jejich produkcí, a to zejména endotypu bronchiálního astmatu spojeného se zvýšenou aktivitou Th2 subsetu a idiopatické plicní fibrózy. V následujícím textu jsou shrnuty některé klíčové poznatky z více než 20 let výzkumu struktury a funkcí periostinu, který přivedl měření jeho sérové hladiny až do klinických studií.
Periostin is a 90kDa matricellular protein, first demonstrated in mouse osteoblasts. Soon after, its presence was demonstrated also in humans. It gained its name due to its high expression in the periosteum. The awareness of pulmonologists and allergists came primarily thr ough the works providing the dependence of its production on stimulation by interleukins 13 (IL-13) and 4 (IL-4) and by transforming growth factor β (TGFβ). This opened the gates to its use as a biomarker of immune pathological conditions associated with their overproduction, especia lly Th2 high asthma endotype and idiopathic pulmonary fibrosis. The following text summarizes some key lessons from about 20 years of resear ch of the structure and functions of periostin, which brought measuring its serum levels up to clinical trials.

We studied the psychophysiology of soluble intercellular adhesion molecule-1 (sICAM-1) in 25 apparently healthy middle-aged men who underwent an acute psychosocial stressor three times with one week apart. Measures of the biological stress response were obtained at week one and three. The magnitude of the sICAM-1 stress response showed no habituation between individual visits. At week one, cognitive stress appraisal independently predicted integrated sICAM-1 area under the curve (AUC) between rest, immediately post-stress, and 45 min and 105 min post-stress (ß=0.67, p=0.012, ?R2=0.41). Diastolic blood pressure AUC (ß=-0.45, p=0.048, ?R2=0.21) and heart rate AUC (ß=0.44, p=0.055, ?R2=0.21) were independent predictors of sICAM-1 AUC at week three. Adjustment for hemoconcentration yielded a decrease in sICAM-1 levels from rest to post-stress (p<0.001). Stress responsiveness of plasma sICAM-1 was predicted by stress perception and hemodynamic reactivity and affected by stresshemoconcentration but unrelated to cortisol reactivity and not readily adapting to repeated stress.

The aim of our study was to evaluate potential differences in the concentration of biochemical markers of endothelial dysfunction between essential hypertension, endocrine hypertension (pheochromocytoma, primary hyperaldosteronism) and control healthy group and to assess a potential relationship between these markers of endothelial dysfunction and vasopressor substances overproduced in endocrine hypertension. We have investigated 21 patients with moderate essential hypertension, 29 patients with primary hyperaldosteronism, 24 subjects with pheochromocytoma and 26 healthy volunteers. Following parameters of endothelial dysfunction were measured, von Willebrand factor (vWf), plasminogen activator (t-PA) and E-selectin (E-sel). Clinical blood pressure was measured according to the European Society of Hypertension recommendations. We found significantly higher levels of the von Willebrand factor in patients with essential hypertension in comparison with a control group (114±20 IU/dl vs 90±47 IU/dl; P=0.04) and patients with primary hyperaldosteronism (114±20 IU/dl vs 99±11 IU/dl; P=0.01). Patients with endocrine hypertension revealed increased levels of vWF compared to the control group, but these differences did not reach statistical significance. Levels of t-PA were increased in patients with pheochromocytoma in comparison with the control group (4.6±1.9 ng/ml vs 3.4±0.9 ng/ml; P=0.01) and with primary hyperaldosteronism (4.6±1.9 ng/ml vs 3.4±1.1 ng/ml; P<0.01). In case of E-selectin we found lower levels in patients with pheochromocytoma in comparison with other groups, but they differed significantly only with primary hyperaldosteronism (40.2±15.0 ng/ml vs 51.3±23.0 ng/ml; P=0.05). Our study did not reveal any convincing evidence of differences in the levels of biochemical markers of endothelial dysfunction between essential and endocrine hypertension. No correlation between the biochemical markers of endothelial dysfunction and vasopressor substances activated in endocrine hypertension was found.

Systémová sklerodermie (SSc) je onemocnění charakterizované postižením cév a zmnožením vaziva kůže a vnitřních orgánů.Pestrost klinických příznaků, chronický průběh a malá výpovědní hodnota běžných reaktantů zánětu jsou hlavními příčinami chybění spolehlivých ukazatelů aktivityonemocnění, které by sloužily k indikaci a monitorování léčby SSc. Cílem studie bylo ověřit, zda by některé z vybraných ukazatelů mohly sloužit ke sledování aktivity SSc. Byly porovnány klinické korelace potenciálních ukazatelů aktivity stanovených u skupiny nemocných SSc při zařazení do studie a při kontrolním vyšetření po 1 roce sledování. Celkem bylo vyšetřeno 49 pacientů – 36 s limitovanou, 9 s difuzní formou a 4 s jinými formami SSc. Byly stanoveny cirkulující hladiny N-terminálního propeptidu prokolagenu typu III (NPIIIP), interleukinu-6 (IL-6), solubilního receptoru pro interleukin- 2 (sIL-2r), intercelulární adhezní molekuly-1 (sICAM-1), vaskulární adhezní molekuly-1 (sVCAM-1), antigenu von Willebrandova faktoru (vWFAg), velkého endotelinu-1 (BET-1) a močová exkrece pyridinolinu (PYR) a deoxypyridinolinu (D-PYR). Potom byly vypočteny korelace těchto markerů s klinickými daty odrážejícími aktivitu a funkčním dotazníkem (FQ). Průměrné hladiny sICAM-1, sVCAM-1, vWFAg, sIL-2r, BET-1 a PYR byly nevýznamně zvýšené ve srovnání s normálními hodnotami. Koncentrace NPIIIP, D-PYR a IL-6 byly normální. Hodnoty naměřené po 1 roce se od vstupního vyšetření významně nelišily. Koncentrace NPIIIP korelovaly se vzdáleností prst – dlaň (FTP) a D-PYR také s FQ. Hladiny IL-6 korelovaly s počtem leukocytů a sIL-2r a s FQ. Kontrolní měření po roce ukázalo stejné korelace NPIIIP a D-PYR. Hladiny IL-6 korelovaly s FQ a sIL-2r s počtem erytrocytů. Koncentrace BET-1 korelovaly s poklesem DLCO. Korelace markerů kolagenního metabolismu s ukazateli kožního postižení a FQ, v souhlase s literárními údaji, dokládá jejich použitelnost při monitorování fibrotických změn. Korelace ukazatelů poškození endotelu a aktivace imunitního systému je nutné ověřit na studiích s větším počtem nemocných.
Systemic scleroderma (SSc) is a disease characterized by vascular damage, skin and internal organ connective tissue thickening or fibrosis. Variability of clinical symptoms, chronic course of the disease and little value of acute-phase reactant measurement, all contribute to the lack of specific markers for the disease activity, which would help with indication of treatment and further follow up of SSc. The aim of this study was to verify if some selected factors could serve as SSc activity markers. Clinical correlations of potential markers of disease activity, determined in a group of SSc patients at the start of the study and after 1 year were compared. In total 49 patients were examined – 36 with limited, 9 with diffuse and 4 with other forms of SSc. The circulating levels of N-terminal propeptide of procollagen type III (PNPIIIP), interleukin-6 (IL-6), soluble interleukin-2 receptor (sIL2-R), intercellular adhesion molecule-1 (sICAM-1), vascular adhesion molecule (sVCAM-1), of von Willebrands factor antigen (vWFAg), big endothelin-1 (BET-1), and urine excretion of pyridinoline (PYR)anddeoxypyridinoline (D-PYR)were determined.Then the correlations of these markers with clinical data reflecting activity and with functional questionnaire (FQ) were measured. The mean levels ofsICAM-1,sVCAM-1,vWFAg,sIL-2R,BET-1 andPYRwerenot significantly elevatedcompared with normal levels. Concentration of NPIIIP, D-PYR and IL-6 were normal. No significant difference was found in the levels of any of these markers at control measurement after 1 year. The level of NPIIIP correlated with the finger to palm (FTP) distance as well as with D-PYR concentration and FQ. IL-6 levels correlated with leukocyte count, sIL-2R and FQ. The same correlations were found for NPIIIP and D-PYR after 1 year. IL-6 levels and FQ, even sIL-2Rand erythrocyte count correlated. BET-1 concentration and decreased DLCO correlated. Correlations of collagen turnover markers with skin damage indicators and with FQ, conforming with literature data, support their possible use for monitoring of fibrotic changes. Correlation of endothelial markers damage with activation of the immune system will have to be verified in larger studies.
