BACKGROUND: Monoamine oxidase (MAO) inhibitors can interact with selective serotonin reuptake inhibitors (SSRIs)/serotonin-norepinephrine reuptake inhibitors (SNRIs). There is clinical interest surrounding use of ozanimod with SSRIs/SNRIs because the major metabolites of ozanimod are weak inhibitors of MAO-B in vitro. OBJECTIVE: To evaluate the incidence of treatment-emergent adverse events (TEAEs) potentially related to serotonin accumulation (SA) during concomitant ozanimod and SSRI/SNRI use by performing analyses of data from an open-label, oral ozanimod 0.92 mg trial (DAYBREAK; NCT02576717). METHODS: SA narrow (serotonin syndrome, neuroleptic malignant syndrome, and hyperthermia malignant) and broad (terms potentially associated with SA) MedDRA v24.0 searches were performed using TEAE data from participants with relapsing multiple sclerosis who entered DAYBREAK from phase 3 studies (cutoff February 1, 2022). Incidences of TEAEs matching terms from each search were stratified by SSRI/SNRI use. RESULTS: Of 2257 DAYBREAK participants, 274 (12.1%) used an SSRI/SNRI. No participants had TEAEs matching the SA narrow search terms. There was no significant difference in the percentage of participants with ⩾1 TEAE matching the SA broad search for those on versus off SSRIs/SNRIs (on: 12.4%, n = 34/274; off: 15.6%, n = 310/1982, nominal p = 0.1630). CONCLUSION: MedDRA searches showed no increase in TEAEs potentially associated with SA with concomitant SSRI/SNRI and ozanimod use.

Antidepresiva představují základní a efektivní terapeutický přístup nejen v léčbě depresivních poruch, ale v rámci komedikace i v řadě jiných neuropsychiatrických onemocnění. V rámci tohoto přehledového článku jsou shrnuty nejčastější nežádoucí účinky nejčastěji používaných antidepresiv v České republice s ohledem na onkologické pacienty a jejich specifické potřeby a zranitelnost. Tyto nežádoucí účinky mají většinou zcela logické vysvětlení na základě jejich receptorového profilu, popř. farmakokinetiky. V tomto kontextu jsou sumarizovány i výhody a nevýhody těchto osmi vybraných nejběžněji používaných antidepresiv u onkologických pacientů především z pohledu snášenlivosti a jejich žádoucích i nežádoucích aditivních efektů.
Antidepressants represent an essential and effective therapeutic approach in treating depressive disorders and many other neuropsychiatric diseases. This review article summarises the most common adverse effects of the most commonly used antidepressants in the Czech Republic with respect to the frailty of cancer patients and their specific needs. These side effects usually have a completely logical explanation based on their receptor profile or pharmacokinetics. In this context, the advantages and disadvantages of these selected most commonly used antidepressants in cancer patients are summarised, especially regarding their tolerability and their desirable and undesirable additive effects.

Serotoninový syndrom je potenciálně fatální komplikace způsobená léčivy či látkami ovlivňujícími serotoninový systém lidského organismu. Může vzniknout v důsledku nasazení léčiva, interakcí mezi léčivy a dalšími substancemi (především návykovými látkami) a konečně také jako důsledek úmyslného či neúmyslného předávkování. Recentní kohortová studie na vzorku 15 milionů pacientů užívajících serotoninergně působící látky udává incidenci v rozmezí 0,07-0,19 za sledované období 4 let. Příznaky serotoninové syndromu dělíme do 3 základních skupin - alterace psychického stavu, autonomní dysregulace a neuromuskulární hyperaktivita. Závažnost průběhu kolísá od mírných příznaků řešitelných vysazením serotoninergní medikace až po závažné stavy vyžadující umělou plicní ventilaci a analgosedaci. Terapie syndromu je dána závažností klinického obrazu, kromě vysazení odpovědné medikace se využívají benzodiazepiny, antikonvulziva a v případě závažnějšího průběhu symptomatická intenzivní péče směřovaná k zajištění vitálních funkcí. S výrazným nárůstem preskripce antidepresiv (především ze skupiny SSRI) souvisí i incidence serotoninového syndromu. I přes dobrý bezpečností profil preskribovaných léků je tedy vhodné mít o existenci serotoninového syndromu povědomí.
Serotonin syndrome is a potentially fatal complication caused by drugs or substances that affect the serotonin system of the human body. It can arise as a result of usage of serotonin-acting medication, interactions with other substances (especially addictive substances) and as a result of intentional or unintentional overdose. A recent cohort study of a sample of 15 million patients taking serotinin-acting agents reported an incidence in the range of 0.07-0.19 over a follow-up period of 4 years. The symptoms of serotonin syndrome are divided into 3 basic groups - alterations of the mental state, autonomic dysregulation and neuromuscular hyperactivity. The severity of the syndrom varies from mild symptoms that can be treated via discontinuation of serotonergic medication to severe conditions requiring lung intensive care unit and sedation. The therapy of the syndrome is given by the state of the patient, in addition to the discontinuation of responsible medication, benzodiazepines and anticonvulsants are also used, and in the case of a more severe course, symptomatic intensive care is aimed to stabilize vital functions. The incidence of serotonin syndrome is also related to a significant increase in prescription of antidepressants (especially SSRI group). Despite the good safety profile of prescribed drugs, it is appropriate to be aware of the existence of serotonin syndrome.

PURPOSE: Recent studies have suggested a lower risk of hepatocellular carcinoma (HCC) in patients receiving selective serotonin reuptake inhibitors (SSRIs). The current study aimed to provide an updated and comprehensive assessment of the association between SSRI use and development of HCC. METHODS: This is a systematic review and meta-analysis of all observational studies published until June 2021. We comprehensively searched PubMed/Medline, Web of Science, and Embase to identify studies comparing SSRIs use with control in relation to the risk of HCC. We calculated pooled relative risks (RRs) and 95% confidence intervals (CIs) for the association between SSRI use and incident HCC risk using random-effects meta-analysis. A dose-response analysis was conducted to evaluate the HCC risk according to the defined daily dose (DDD) of SSRI use. RESULTS: Eight observational studies, comprising 1,051,096 participants and 22,316 incidences of HCC, examining the association between SSRIs use and HCC risk, were included in the systematic review (adjusted RR: 0.66; 95% CI: 0.56-0.79; P ≤ 0.001). In subgroup analysis, the magnitude of benefit associated with SSRIs was significantly higher in patients with hepatitis infection (RR: 0.70, 95% CI: 0.51-0.95) than the general population (Pheterogeneity = 0.700). The dose-response analysis indicated strong inverse association between cumulative DDD of SSRI and risk of HCC (coefficient: - 0.0030; P = 0.002; R2 = 0.78). CONCLUSIONS: The results of this review show that SSRI use was associated with a 34% lower risk of HCC, which tend to be dose dependent. Further prospective studies are warranted to confirm these observations across the spectrum of chronic liver disease and hepatitis infection.

BACKGROUND: Painful diabetic neuropathy is an important therapeutic challenge as the efficacy of analgesic drugs in this setting is still unsatisfactory. Monotherapy with available treatments is often not sufficient and a combination of drugs is necessary. Trazodone (TRZ) is a compound with a multi-modal mechanism of action, being a serotonin-2 antagonist/reuptake inhibitor developed and approved for the treatment of depression in several countries. Previous clinical trials suggest a possible beneficial effect of low doses of trazodone for the treatment of patients affected by painful diabetic neuropathy. OBJECTIVE: This phase II study was designed to collect data on the efficacy and safety of low doses of TRZ combined with gabapentin after 8 weeks of treatment in patients affected by painful diabetic neuropathy. METHODS: This was a randomized, double-blind, placebo-controlled, multi-center, international, prospective study. Male and female diabetic patients aged 18-75 years and affected by painful diabetic neuropathy were eligible for enrollment. Subjects were randomized (1:1:1 ratio) to TRZ30 (10 mg three times daily for 8 weeks) or TRZ60 (20 mg three times daily for 8 weeks) or placebo. Gabapentin as background therapy was administered in open-label conditions to all patients. The primary endpoint was the change from baseline of the Brief Pain Inventory Short Form item 5 to week 8. Secondary endpoints included the other Brief Pain Inventory Short Form items, and the assessment of anxiety, sleep, quality of life, patient's improvement, and safety. RESULTS: One hundred and forty-one patients were included in the intention-to-treat population: 43 allocated to the TRZ30 group, 50 to the TRZ60 group, and 48 to the placebo group. After 8 weeks, the mean changes of Brief Pain Inventory Short Form item 5 from baseline were - 3.1, - 2.6, and - 2.5 in the TRZ30, TRZ60, and placebo groups, respectively. No statistically significant differences between groups were seen. Nevertheless, a better trend was observed for TRZ30 vs placebo (95% confidence interval - 1.30, 0.15; p = 0.1179), on top of the background effect of gabapentin administered to all study groups. 62.8% of patients achieved a ≥ 50% reduction in the TRZ30 group, 54% in the TRZ60 group, and 45.8% in the placebo group. At the same time, a statistically significant improvement was observed in Brief Pain Inventory Short Form item 6 for TRZ30 vs placebo (95% confidence interval - 1.54, - 0.07; p = 0.0314). No serious adverse event occurred during the trial and the most frequent treatment-emergent adverse events involved nervous system, QT prolongation, and gastrointestinal disorders. CONCLUSIONS: All treatment groups showed a clinically meaningful pain improvement; nevertheless, patients in the TRZ30 treatment group reported better efficacy outcomes. This finding suggests that low doses of TRZ could be useful for treating painful diabetic neuropathy, and support further adequately powered confirmatory trials investigating the efficacy of TRZ. U třetiny pacientů po cévní mozkové příhodě (CMP) se rozvine klinicky významná deprese (PSD). Vyšší riziko je u pacientů s depresivní poruchou v anamnéze, rozsáhlejší CMP a výraznějším narušením motorických a kognitivních funkcí. S ohledem na spojení mezi PSD a vyšší mortalitou, horším výsledným fungováním a úrovní motorického a kognitivního postižení, je včasné rozpoznání PSD a její léčba zásadní krok v péči o pacienty po CMP. Dosavadní studie naznačují jak účinnost antidepresiv (AD) v léčbě PSD, tak efekt na celkové fungování včetně zlepšení motorického deficitu, podobně se AD jeví jako potenciálně použitelná preventivní intervence. První volbou zůstávají AD skupiny SSRI, je však nutno počítat s iniciálními nežádoucími účinky a možnými lékovými interakcemi a reflektovat možné zvýšené riziko krvácivých příhod. Jiná antidepresiva jsou buď spojena s vyššími riziky (tricyklická AD) nebo nejsou dostatečně prověřena (novější AD). Vzhledem k dobrému bezpečnostnímu profilu, účinnosti u starších osob a efektu na kognitivní funkce se zajímavou alternativou k SSRI jeví vortioxetin. Psychosociální intervence, minimálně zaměřené na edukaci o PSD, podpoře pečovatelů a zvyšování adherence k léčbě jsou nedílnou součástí intervencí a v kombinaci s AD zvyšují šanci na uspokojivý výsledek. Stimulační metody jsou aktuálně rezervované pro závažné případy PSD nebo jako alternativa AD, nicméně důkazy o účinnosti (a u ECT bezpečnosti) jsou limitované.
One third of stroke survivors develop a clinically significant depression (poststroke depression, PSD). A higher risk is in patients with a history of depressive disorder, a more severe stroke and a more pronounced impairment of motor or cognitive functions. Given the link between PSD and higher mortality, worse outcomes, and a more pronounced motor and cognitive impairment, early recognition of PSD and its treatment is a crucial step in a care after stroke. Previous studies suggest the efficacy of antidepressants (ADs) in both the treatment of PSD and the effect on daily functioning including alleviation of motor impairment, and a possible usefulness of ADs for prophylaxis of PSD. SSRIs remain the first choice in PSD, but initial side effects, drug interactions, and the possible increased risk of bleeding events should be considered. Other ADs are either associated with higher risks (tricyclic ADs) or have not sufficiently proven their effect (newer AD) in PSD. Among them, vortioxetine seems to be an interesting alternative to SSRIs due to its good safety profile, efficacy in the elderly, and an effect on cognitive function. Psychosocial interventions providing at least an education about PSD, a support for caregivers, and strategies to improve adherence to treatment should be an integral part of collaborative care intervetions to improve treatment outcome. Stimulation methods are currently reserved for severe PSD cases or as an alternative to ADs, but evidence of efficacy (and of a safety concern in ECT) is limited.
