De novo sequence variants, including truncating and splicing variants, in the additional sex‑combs like 3 gene (ASXL3) have been described as the cause of Bainbridge‑Ropers syndrome (BRS). This pathology is characterized by delayed psychomotor development, severe intellectual disability, growth delay, hypotonia and facial dimorphism. The present study reports a case of a girl (born in 2013) with severe global developmental delay, central hypotonia, microcephaly and poor speech. The proband was examined using a multi‑step molecular diagnostics algorithm, including karyotype and array‑comparative genomic hybridization analysis, with negative results. Therefore, the proband and her unaffected parents were enrolled for a pilot study using targeted next‑generation sequencing technology (NGS) with gene panel ClearSeq Inherited DiseaseXT and subsequent validation by Sanger sequencing. A novel de novo heterozygous frameshift variant in the ASXL3 gene (c.3006delT, p.R1004Efs*21), predicted to result in a premature termination codon, was identified. In conclusion, the present study demonstrated that targeted NGS using a suitable, gene‑rich panel may provide a conclusive molecular genetics diagnosis in children with severe global developmental delays.

The TRPS1 protein is a potent regulator of proliferation, differentiation, and apoptosis. The TRPS1 gene aberrations are strongly associated with rare trichorhinophalangeal syndrome (TRPS) development. We have conducted MLPA analysis to capture deletion within the crucial 8q24.1 chromosomal region in combination with mutation analysis of TRPS1 gene including core promoter, 5'UTR, and 3'UTR sequences in nine TRPS patients. Low complexity or extent of untranslated regulatory sequences avoided them from analysis in previous studies. Amplicon based next generation sequencing used in our study bridge over these technical limitations. Finally, we have made extended in silico analysis of TRPS1 gene regulatory sequences organization. Single contiguous deletion and an intragenic deletion intervening several exons were detected. Mutation analysis revealed five TRPS1 gene aberrations (two structural rearrangements, two nonsense mutations, and one missense substitution) reaching the overall detection rate of 78%. Several polymorphic variants were detected within the analysed regulatory sequences but without proposed pathogenic effect. In silico analysis suggested alternative promoter usage and diverse expression effectivity for different TRPS1 transcripts. Haploinsufficiency of TRPS1 gene was responsible for most of the TRPS phenotype. Structure of TRPS1 gene regulatory sequences is indicative of generally low single allele expression and its tight control.

[Classic form of Ehlers-Danlos syndrome]
Vzácná onemocnění, Ehlersovy-Danlosovy syndromy, patří do skupiny dědičných chorob pojivové tkáně. Projevují se hypermobilitou kloubů, zvýšenou elasticitou a křehkostí kůže, vznikem pseudotumorů a zejících jizev. Původní klasifikace podle Villefranche (1997) dělila syndrom Ehlersův-Danlosův (EDS) do 6 skupin. Nová klasifikace 2017 vychází ze současných poznatků molekulární diagnostiky a rozlišuje 13 subtypů Ehlersových-Danlosových syndromů. Autoři prezentují čtyřletého pacienta s klasickým subtypem EDS, který byl potvrzen histologickým vyšetřením kůže pacienta a DNA analýzou, při které byla zjištěna dosud nepopsaná mutace (patogenní sekvenční varianta) c.1048insG p.Ser350Cysfs*4 v 7. exonu genu COL5A1. Současně představují novou klasifikaci.
Ehlers-Danlos syndromes belong to the group of inherited connective tissue disorders characterized by increased skin elasticity, joint hypermobility, cutaneous fragility with formation of pseudotumors and gaping scars. Original classification by Villefranche (1997) divided Ehlers-Danlos syndrome into 6 groups. New classification (2017) supported by molecular analysis distinguishes 13 subtypes of Ehlers-Danlos syndromes. We present a case of 4-year old boy with classic EDS confirmed by histological skin examination and DNA analysis revealing yet non- published mutation (sequence variant) c.1048insG p.Ser350Cysfs*4 in the 7th exon of the COL5A1 gene.

Sekvenování lidského genomu umožnilo identifikovat jednotlivé lidské geny, stanovit variace těchto genů a pochopit jejich přispění ke zdraví a nemoci. Zároveň s technologickým pokrokem dochází ke zlevňování a zpřístupnění genetického testování. Tyto skutečnosti posouvají lékařskou genetiku od diagnostiky vzácných nemocí u dětí a prenatální diagnostiky směrem ke všem věkovým skupinám. Z toho vyplývá nutnost integrace poznatků lékařské genetiky do interních oborů a do ambulancí praktických lékařů. Existence alternativních forem genu (alel) v populaci, výskyt podobných fenotypů podmíněných mutacemi a variabilitou v různých lokusech, význam genových interakcí a interakcí genů a prostředí u lidských onemocnění, úloha somatických mutací v karcinogenezi a stárnutí, možnost prenatální diagnostiky, presymptomatické testování a populační screening, příslib účinných genových terapií, to vše jsou principy, které již nyní pronikají do všech oblastí medicíny. Mezinárodní kolaborativní studie prokazují, že v současné době potřebuje 10 % populace v některém období života genetické zdravotnické služby. Toto procento se bude nepochybně v budoucnosti, s dalším nárůstem zejména molekulárně genetických poznatků a možností zvyšovat. Míra pokroku využití genetického přístupu v celém kontinuu zdravotnické péče o pacienta a jeho rodinu závisí nejenom na technologickém pokroku, ale i na odborné přípravě ošetřujících lékařů.
The sequencing of the human genome made it possible to identify individual human genes, to determine variations of these genes and to understand their contributions to health and diseases. With technological progress, there came cheaper and more accessible genetic testing. This changes medical genetics from being almost solely focused on diagnosing rare diseases in children and prenatal diagnostics to something available to all age groups. Hence the need for integration of information from and about the field of medical genetics into internal medicine and general practitioners’ everyday work. The existence of alternative forms of a gene (alleles) in the population, the occurrence of similar, mutation-conditional phenotypes and variability in different loci, the importance of gene interactions and gene-environment interactions in human diseases, the role of somatic mutation in carcinogenesis and aging, the possibility of prenatal diagnosis, non-symptomatic testing and popula- tion screening, the promise of effective gene therapies – all these are ideas and principles that now permeate all areas of medicine. International collaborative studies show that some 10 % of the population currently require genetic health services at some point in their lives. This percentage will undoubtedly rise in the future, especially with further increases of our knowledge of molecular genetics. Progress towards the use of genetic approach throughout the continuum of health care for patients and their families depends not only on technological progress, but also on proper training of active physicians.
PURPOSE: The aim of this study was to analyze homocysteine, folate and cobalamin in men with normozoospermia, obstructive and non-obstructive azoospermia. METHODS: Analysis of plasma and seminal plasma homocysteine, folate and cobalamin in 72 azoospermic and 62 normozoospermic men. Evaluation of the azoospermic patient included testicular biopsy, endocrine, urological and ultrasound examination. RESULTS: Homocysteine (1.2 μmol/l) and cobalamin (322.05 pmol/l) concentrations (median values) in seminal plasma were significantly lower (p < 0.001) in men with azoospermia than in men with normozoospermia (2.5 μmol/l and 579.0 pmol/l). Folate and cobalamin concentrations were significantly higher in obstructive than in non-obstructive azoospermia. Significant correlations were determined between testis volume and seminal plasma homocysteine in azoospermic men. CONCLUSION: Lower concentrations of homocysteine and cobalamin (but not folate) were found in azoospermic seminal plasma than normozoospermic. Folate and cobalamin were higher in seminal plasma from obstructive azoospermia than in non-obstructive azoospermia patients.

This study was designed to evaluate the use of sperm frozen before testicular cancer treatment and to analyse results of infertility treatment. A total of 166 male patients were referred for sperm freezing before testicular cancer treatment from 1995 to 2007. Cancer treatment, semen analysis, assisted reproduction techniques, and statistical evaluation were basic interventions. Sperm pathology analysis before and after cancer treatment, survival of patients, and pregnancy after infertility treatment were analysed. Testicular cancer was diagnosed and treated in 226 men from 1995 to 2007. A total of 166 men (73.5 %) decided to freeze their semen before cancer treatment. Seminoma was diagnosed in 87 men (52.4 %), nonseminomatous germ cell tumours (NSGCT) in 79 men (47.6 %). A total of 121 men (72.9 %) were of stage I. Azoospermia was diagnosed in 9 men (5.4 %), semen was cryopreserved in 157 patients. Median sperm concentration was 8.5 mil/ml in the seminoma group and 9.4 mil/ml in the NSGCT group – a non-significant difference. Until now, 5 patients (2.9 %) have died and 27 patients (17.2 %) have attended for infertility treatment. Median fresh sperm concentration was 4.4 mil/ml, median progressive sperm motility 3.7 %, azoospermia was found in 4 men (14.8 %). The confounding female infertility factor was found in 19 (70.4 %) female partners. Cryopreserved semen was used in 25 couples (92.6 %), intracytoplasmic sperm injection (ICSI) was the most effective procedure – 36 cycles resulted in 12 pregnancies and 9 deliveries. Testicular cancer survivors have a good chance of fathering a child by using cryopreserved sperm and ICSI procedure.

Kauzistické sdělení pojednává o embolizaci spermatické žíly, která je v pravostranné lokalitě prováděna zcela ojediněle. Autoři předkládají případ 27letého muže s pravostrannou varikokélou a intenzivně vzniklými orchialgiemi, neřešitelného bezpečně operativní technikou. Endovaskulární řešení vedlo k ústupu bolestivosti i regresi varikokély.
Case report describes embolisation of spermatic vein as very unusual procedure on the right side. Authors are showing case report of 27 years old man with varicocele and severe scrotal pain, which could not be treated by surgery. After endovascular radiological embolisation the variococele decreased in size and scrotal pain disappeared as well.
