The precise method of action of dienogest on the production and development of endometriosis lesions is unknown, and its controversial effect on endometrial thickness has been under investigation. In the following study the Dienogest's effects on the target animal's histology of female reproductive organs, including the tissues from the Fallopian tubes, ovaries, and uterus, as well as the impact of drug administration on the liver's enzymes and the drug's effects on triglycerides, body weight, and HbA1c, have all been studied. The findings of the following experiment indicated that there was no significant elevation of liver enzymes. The little to no elevation of the liver enzymes indicated that the drug did not induce stress on the hepatic cells and according to the subsequent experiment it is safe for clinical use. Moreover, after 10, 20, and 30 days of blood administration, the level of blood TG significantly decreased, and after 30 days of intake, the level of blood sugar significantly decreased. However, there were no appreciable changes after 10 and 20 days. After 30 days of treatment, the rats' weight also showed a very minor drop. In addition to it, the results of histological changes in the tissue in the following study represented that there were evident changes in the tissues which comprised of decline in blood circulation, fibrosis in tissues, and degeneration of follicles.

The use of hormonal contraception (HC) has been increasing in recent decades. In women with migraine, the use of HC can significantly positively as well as negatively affect the course of the disease. In women with vascular risks, the use of inappropriate HC can make them more likely to develop ischaemic stroke. In migraine without aura, it is possible to prescribe both combined and gestagen-only HC; in migraine with aura, it is the gestagen-only type only. In menstrual migraine, in order to reduce menstruation-related migraine attacks, extended-cycle or continuous-cycle combined oral contraceptives (COCs), or gestagen-only HC can be administered. COCs are contraindicated in women with a risk of ischaemic stroke and in those with a history of ischaemic stroke. In women with risk factors of ischaemic stroke, but with no history of ischaemic stroke, it is possible to administer gestagen-only HC, with this type being relatively contraindicated after having ischaemic stroke.
Užívání hormonální antikoncepce (HAK) v posledních desetiletích narůstá. U žen s migrénou může užívání HAK významně pozitivně, ale i negativně ovlivnit průběh onemocnění. U žen s cévními riziky může vést užívání nevhodné HAK k vyšší pravděpodobnosti rozvoje ischemické cévní mozkové příhody (iCMP). U migrény bez aury je možno nasadit jak kombinovanou, tak čistě gestagenní HAK, u záchvatů s aurou pouze čistě gestagenní typ. U menstruační migrény lze za účelem snížení záchvatů vázaných na menses podat kombinovanou orální antikoncepci (COC) v rozšířeném cyklu, resp. kontinuálně, nebo nasadit čistě gestagenní HAK. U žen s rizikem iCMP a po proběhlé iCMP je COC kontraindikována. U žen s rizikovými faktory pro CMP, u nichž CMP neproběhla, je možno podat čistě gestagenní HAK, po proběhlé iCMP je tento typ relativně kontraindikován.

BACKGROUND: A recent study focusing on dietary predictors of nausea and vomiting in pregnancy (NVP) found that women with higher levels of partner support, and those who had used oral contraception (OC) when they met the father, both tended to report less severe NVP compared with previous non-users or those with less supportive partners. We provide a further test of these factors, using a large sample of women from four countries who retrospectively scored their NVP experience during their first pregnancy. METHODS: We recruited women who had at least one child to participate in a retrospective online survey. In total 2321 women completed our questionnaire including items on demographics, hormonal contraception, NVP, and partner support. We used general linear models and path analysis to analyse our data. RESULTS: Women who had used OC when they met the father of their first child tended to report lower levels of NVP, but the effect size was small and did not survive adding the participant's country to the model. There was no relationship between NVP and partner support in couples who were still together, but there was a significant effect among those couples that had since separated: women whose ex-partner had been relatively supportive reported less severe NVP. Additional analyses showed that women who were older during their first pregnancy reported less severe NVP, and there were also robust differences between countries. CONCLUSIONS: These results provide further evidence for multiple influences on women's experience of NVP symptoms, including levels of perceived partner support.

Cíl: V České republice lze ženě uměle ukončit těhotenství farmakologickou metodou (UUT-F) v I. trimestru do 49. dne sekundární amenorey. Cílem studie je analýza významu stanovení lidského choriového gonadotropinu (hCG) v séru/moči a ultrazvukového (UZ) vyšetření při diagnostice těhotenství a hodnocení výsledku UUT-F. Metodika: V letech 2017 a 2018 bylo na našem pracovišti provedeno UUT-F celkem u 109 žen sekvenčním podáním mifepristonu (600 mg perorálně) a misoprostolu (400 mcg perorálně). Stanovení hCG v séru/moči a UZ vyšetření byly provedeny při diagnostice těhotenství i hodnocení výsledku UUT-F. Výsledky: Při diagnostice těhotenství byla pozitivní a středně silná korelace mezi hCG v séru a průměrem gestačního váčku v dutině děložní (GS – gestational sac) (r = 0,711; p < 0,0001) i velikostí zárodku v dutině děložní (CRL – crown-rump length) (r = 0,605; p < 0,0001). Délka těhotenství byla 42–49 dní amenorey (průměr 45,6; medián 45), ženy byly ve věku 16–44 let (průměr 29,4; medián 29). Při kontrolním vyšetření po UUT-F byla u 13,8 % žen (15/109) hodnota hCG v séru > 1 000 IU/l a u 17,4 % (20/109) byl pozitivní low sensitivity urine pregnancy test (LSUP test). UZ vyšetření diagnostikovalo „Pokračující těhotenství“ u pěti žen a zamlklé těhotenství u jedné ženy (hCG v séru bylo vždy > 1 000 IU/l a LSUP test byl vždy pozitivní). U 5,5 % žen (6/109) byla provedena následná chirurgická intervence vč. „Pokračujícího těhotenství“ (n = 5); u zamlklého těhotenství (n = 1) byl podán další misoprostol a chirurgickou intervenci nebylo nutné provést. Závěr: Při diagnostice těhotenství je pozitivní a středně silná korelace mezi hCG v séru a CRL. Při kontrolním vyšetření po UUT-F umožní negativní LSUP test spolehlivě vyloučit pokračující těhotenství. V případě pozitivního LSUP testu by mělo být provedeno ultrazvukové vyšetření, nicméně chirurgická intervence by neměla být indikována pouze na základě rozšíření dutiny děložní.
Objective: In the Czech Republic, it is possible, to carry out Medical Termination of Pregnancy (MToP) in the 1st trimester up until the 49th day of secondary amenorrhea. The aim of the study is to analyse the significance of serum/urine human chorionic gonadotropin (hCG) assessment and ultrasound (US) examination in pregnancy diagnosis and MToP follow-up. Methods: In 2017–2018, MToP was carried out in a total of 109 women by administering a combination of mifepristone (600 mg orally) and misoprostol (400 mcg orally). Serum/urine (LSUP – low sensitivity urine pregnancy test) hCG assessment and US examination were performed at pregnancy diagnosis and MToP follow-up. Results: At pregnancy diagnosis, there was a positive and medium strong correlation between serum hCG and size of the gestational sac – GS (R = 0.711; P < 0.0001) and crown-rump length of the embryo – CRL (R = 0.605; P < 0.0001). Gestational age was 42–49 days (average 45.6, median 45 days), the women were 16–44 years of age (average 29.4, median 29 years). In MToP follow-up, serum hCG level was > 1,000 IU/L in 13.8% of women (15/109) and a positive LSUP test in 17.4% (20/109). US examination diagnosed ongoing pregnancy in five women and missed abortion in one woman (serum hCG was always > 1,000 IU/L and LSUP test was always positive). In 5.5% of women (6/109), a subsequent surgical intervention was carried out including those with ongoing pregnancy (N = 5); missed abortion (N = 1) was treated by additional misoprostol, where surgical intervention was not necessary. Conclusion: At pregnancy diagnosis, there is a positive and medium strong correlation between serum hCG and CRL. In MToP follow-up, a negative LSUP test enables reliable exclusion of ongoing pregnancy and missed abortion. In case of a positive LSUP test, US examination should be performed; however, surgical intervention should not be indicated solely on the basis of uterine cavity dilatation.

Cíl: Navzdory značným pokrokům s nedávno vyvinutými formami kombinované perorální antikoncepce (COC – combined oral contraceptive), které vedly ke sníženému výskytu nežádoucích příhod při plném zachování antikoncepční účinnosti, zájem o další inovace přetrvává. Materiály a metody: Nový typ COC kombinuje přirozený estrogen estetrol (E4) a gestagen drospirenon (DRSP). Evropský panel odborníků hodnotil farmakologické vlastnosti, účinnost, bezpečnost a snášenlivost této kombinace. Zjištění jsou prezentována formou přehledového článku. Výsledky: Kombinace 15 mg E4/3 mg DRSP v režimu 24+4 představuje účinnou antikoncepci s dobrou regulací cyklu, charakterizovanou pravidelným krvácením a minimálním neplánovaným krvácením, a stejně tak i dobrým bezpečnostním profilem. Spojován je s vysokou spokojeností uživatelek, dobrým zdravotním stavem a minimálními změnami tělesné hmotnosti. Účinky na endokrinní a metabolické parametry jsou omezeny a kombinace má též omezený vliv na jaterní funkce a metabolizmus lipidů a sacharidů. Na hemostatické parametry má menší vliv než porovnatelná léčiva s obsahem 20 μg etinylestradiolu (EE) /3 mg DRSP a 30 μg EE/150 μg levonorgestrelu. Závěr: Kombinace 15 mg E4/3 mg DRSP poskytuje bezpečnou a účinnou antikoncepci s vysokou mírou spokojenosti uživatelek a předvídatelným krvácením. Další výzkum se bude věnovat hodnocení dlouhodobé bezpečnosti COC.
Purpose: Despite considerable advances in recently developed combined oral contraceptives (COCs), resulting in lower rates of adverse events while maintaining contraceptive efficacy, there is interest in further innovation. Materials and Methods: Estetrol (E4), a native oestrogen, and progestin drospirenone (DRSP) were combined in a new COC. A European expert panel reviewed the pharmacology, efficacy, and safety and tolerability of this combination. Their findings are presented as a narrative review. Results: E4 15mg/DRSP 3 mg in a 24/4 regimen provided effective contraception with good cycle control, characterised by a predictable regular bleeding pattern and minimal unscheduled bleeding, together with a good safety profile. The combination was associated with high user satisfaction, wellbeing, and minimal changes in body weight. The effects on endocrine and metabolic parameters were limited, and the combination was found to have a limited impact on liver function and lipid and carbohydrate metabolism. Moreover, its effect on several haemostatic parameters was lower than that of comparators containing ethinyl oestradiol (EE) 20 mg/DRSP 3 mg and EE 30 mg/levonorgestrel 150 mg. Conclusion: E4 15 mg/DRSP 3 mg provides safe and effective contraception, with high user satisfaction and predictable bleeding. Further research will evaluate the long-term safety of the COC.
