Histologické vyšetření nenádorových biopsií žaludku patří mezi rutinní činnosti patologa. Většinu těchto biopsií představují gastritidy a gastropatie. Ačkoli základním klinickým požadavkem je dosud v těchto případech potvrzení či vyloučení přítomnosti helicobacterové infekce, patolog musí mít na paměti celé spektrum příčin a asociovaných morfologických obrazů. Ty sestávají z kombinací zánětlivé infiltrace různého charakteru, alterace a reaktivních změn epitelové komponenty, odpovědi vaskulárního aparátu a odlišností ve složení slizničního stromatu. Asociaci histopatologického obrazu s předpokládanou příčinou změn lze někdy podpořit i přímým průkazem etiologického agens ve vyšetřovaném endoskopickém vzorku.
Histological investigation of non-neoplastic endoscopic biopsies of gastric mucosa is one of the most common tasks most pathologists have to face on daily basis. Although the most common clinical question is still being whether Helicobacter organisms are found, pathologists have to bear in mind the whole spectrum of causes and associated morphological patterns of gastritides and gastropathies, governed by characteristic combinations of various types of inflammatory infiltrate, alterative and reactive changes of epithelial component, vascular response, and variability of stromal composition. The association of histopathologic pattern with supposed etiology can be sometimes proved by direct detection of the cause of morphologic changes in the investigated endoscopic sample.

STUDY AIMS: Small bowel capsule endoscopy (SBCE) is the first line procedure for detecting small bowel lesions in patients with an obscure gastrointestinal bleeding (OGIB). Missed upper gastrointestinal (UGI) lesions at the initial endoscopy may account for the so-called OGIB. This retrospective study was designed to assess the role of SBCE in detecting missed UGI lesions. METHODS: All consecutive SBCE that were performed in the last year for patients with OGIB were included in our study. We evaluated the visibility of the gastric mucosa, the anatomic landmarks, the presence of UGI lesions as well as their clinical importance. The SBCE findings were compared with the reports of previous UGI endoscopies. RESULTS: 118 patients (45 males, 73 females, mean age 61 +/- 19 years) were included in the analysis. The indication for SBCE was obscure overt and occult OGIB in 60 and in 58 patients, respectively. SBCE identified lesions in the small bowel in 42% of the patients. An excellent visibility of gastric mucosa was observed in 83/118 cases (703%). SBCE identified gastric lesions with potential clinical significance (high bleed potential) in 25/118 (21.2%) patients. In 12/118 (10.2%) patients the UGI lesions detected by SBCE were considered as the only potential source of bleeding. CONCLUSIONS: In patients with OGIB, SBCE detected not only small bowel lesions but also significant UGI lesions that were missed or underestimated at the initial endoscopy in 21% of cases. It is therefore necessary to carefully read the gastric images when performing an SBCE.

Heterotopický pankreas je definovaný ako prítomnosť abnormálne lokalizovaného pankreatického tkaniva bez akejkoľvek anatomickej a cievnej kontinuity s normálnym pankreasom. Incidencia lézie je 0,2 až 0,5 % pacientov podstupujúcich hornú mediálnu laparotómiu. Najčastejším symptómom heterotopického pankreasu je diskomfort a bolesť brucha, krvácanie z hornej časti tráviaceho traktu a intermitentný ileózny stav. Ako diagnostická metóda prvej voľby je gastrofibroskopické vyšetrenie. Ako zobrazovacie metódy sú použiteľné ultrasonografické vyšetrenie, počítačová tomografia, no najmä endoskopické ultrasonografické vyšetrenie. Prezentovaná je kazuistika muža so symptomatológiou krvácania z hornej časti tráviaceho traktu. Zobrazovacie vyšetrenia poukazovali na tumorózny proces steny žalúdka veľkosti 3 cm. Pacient podstúpil chirurgickú resekciu žalúdka podľa Billrotha II. Histologické vyšetrenie dokázalo prítomnosť heterotopického pankreasu typu 1 Heinrichovej klasifikácie. Vzhľadom na submukóznu lokalizáciu je predoperačná histologická diagnóza heterotopického pankreasu obtiažna. Vo väčšine prípadov ju dovoľuje len tenkoihlová biopsia cestou gastrofibroskopie. Pri úspešnom histologickom vyšetrení a vhodnej lokalizácii je možné endoskopické odstránenie lézie. Vo väčšine prípadov je však nutná chirurgická excízia ložiska. Peroperačné histologické vyšetrenie dovoľuje minimalizáciu rozsahu resekcie. Názory na liečbu asymptomatického heterotopického pankreasu sú nejednoznačné. Väčšina publikovaných prác však doporučuje excíziu lézie za účelom vyhnúť sa ďalším komplikáciám.
Heterotopic pancreas (HP) is defraed as abnormally localized pancreatic tissue without any anatomical and vascular connection with pancreas. Incidence of HP is 0.2–0.5% of patients underwent upper medial laparotomy. The most common symptoms of HP are abdominal discomfort and pain, upper gastrointestinal bleeding and intermittent bowel obstruction. Gastroscopy is the most commonly used first diagnostic procedure. Ultrasound evaluation, computer tomography and namely endoscopic ultrasound evaluation are useful imaging procedures of HP. Casuistic report of patient with upper gastrointestinal bleeding is presented. Imaging procedures have recorded stomach tumor 3cm in diameter in back wall of stomach. Billroth II resection of stomach was performed. Histological evaluation of removed part of stomach recorded heterotopic pancreas type 1 of Heinrich classification. HP diagnosis before surgery is difficult because of submucous localization of lesion. Fine needle biopsy during gastroscopy is one of diagnostic possibilities without surgery. Endoscopic excision of lesion is possible if histological evaluation by fine needle biopsy is successful and anatomical localization is appropriate. The most of patients need surgical excision of HP. Peroperative histological evaluation allows a minimalization of excision area. Opinions of asymptomatic HP treatment are ambiguous. The most of published papers recommend excision of asymptomatic HP also, because of risk of next complications.

Vanek's tumor (inflammatory fibroid polyp) is a rare benign lesion occurring throughout the digestive tract. Histologically, two patterns can be recognized. Classical Vanek's tumor contains concentric formations of proliferating spindle cells which are CD34 positive. Atypical, inflammatory pseudotumor-like Vanek's tumor lacks concentric formations and is CD34 negative. Recently, mutations in platelet-derived growth factor receptor alpha (PDGFRA) were reported in gastric and small intestinal Vanek's tumors. In this study, KIT exons 9, 11, 13, and 17, PDGFRA exons 12, 14, and 18, and a part of exon 15 BRAF for point mutation V600E were screened in 23 cases of Vanek's tumor, both classical (n = 16) and inflammatory pseudotumor-like (n = 7). No mutations in all analyzed exons of KIT and BRAF and in exon 14 of PDGFRA were detected. Six Vanek's tumors harbored activating mutations in PDGFRA exons 12 (n = 5) and 18 (n = 1) respectively: S566_E571delinsK (n = 1), S566_E571delinsR (n = 4), and D842 del (n = 1). The mutations were detected in the classical (n = 5), as well as inflammatory pseudotumor-like (n = 1) Vanek's tumors. The results of this study suggest that the two morphological patterns of Vanek's tumor more probably represent only variants of one type of tumor than two different lesions. Furthermore, BRAF mutations were not shown to drive growth of PDGFRA wild-type Vanek's tumors.

Autoři předkládají kazuistiku 20letého muže, který utrpěl polytrauma při autonehodě (ISS 22, kontuze mozku, kontuze plic, sériová fraktura žeber, fraktura Th8, hematom levé ledviny s hematurií). Operace nebyla indikována, hrudník i břicho byly opakovaně v průběhu hospitalizace i následné dispenzarizace rentgenologicky, sonograficky a CT vyšetřeny. Expanze žaludku v pleurální dutině v časovém odstupu imitovala klinický i rentgenologický nález typický pro tenzní pneumotorax. Urgentní drenáž ukončila kardiopulmonální selhávání a zároveň vyjasnila diagnózu. K neobvyklému případu je podán krátký literární přehled.
The authors present a case-review of a 20-year-old male who suffered from a car accident polytruma (the ISS 22, brain contusion, lung contusion, serial ribs fructures, fracture of the Th8, haematoma of the left kidney with haematuria). Surgery was not indicated, his thorax and abdomen were repeatedly examined with x-rays, ultrasound and CT scans, during the patient‘ s hospitalization and during the follow-up period. After some time, the stomach expansion in the pleural cavity imitated the clinical and roentgenological findings typical for the tension pneumothorax. Urgent drainage solved the cardiopulmonary failure and cleared up the diagnosis. This atypical case is followed by a short literature overview.
