Prolaktinomy jsou nejčastější hormonálně aktivní adenomy hypofýzy. U žen je obvyklým příznakem onemocnění amenorea či jiná porucha menstruace. Diagnostika je založena na zjištění hyperprolaktinemie s verifikací adenomu v oblasti tureckého sedla pomocí magnetické rezonance. Ve většině případů má dobrý efekt samostatná farmakoterapie kabergolinem. Léčbu makroprolaktinomů je nutné individuálně zvažovat, uplatňuje se kombinace více modalit, například operace nebo ozáření gama nožem následované farmakoterapií.
Prolactinomas are the most common hormonally active pituitary adenomas. For women, amenorrhea or other menstrual disturbances are common symptoms of the disease. Diagnosis is based on the detection of hyperprolactinaemia with verification of the adenoma in the region of the Turkish saddle using magnetic resonance imaging. In most cases, cabergoline alone has a good effect. The treatment of macroprolactinomas must be individually considered; a combination of several modalities, such as surgery or gamma-knife irradiation followed by drug therapy, is applied.

Při podávání antipsychotik je v literatuře věnován dostatek prostoru jejich účinnosti na pozitivní příznaky. Často jsou diskutovány jejich kardiovaskulární a metabolické nežádoucí účinky a vhodná doporučení k jejich korekci. Recentně je hojně prostoru věnováno i vlivu antipsychotik na negativní příznaky. Téma hyperprolaktinémie a zejména osteoporózy ve vztahu k antipsychotické léčbě je v české literatuře méně časté. Je však pravděpodobné, že zlepšováním léčby psychóz se nemocní dožijí vyššího věku, a tím více se mohou potýkat s následky polékové hyperprolaktinémie včetně osteoporózy. Léčba antipsychotiky mnohdy způsobuje vzestup sérového prolaktinu, což může vést k hypogonadismu. To pak přímo či nepřímo ovlivňuje kostní metabolismus. Schopnost jednotlivých antipsychotik ovlivňovat prolaktinémii pak může být vodítkem v redukci rizika osteoporózy vlivem dlouhodobé léčby.
When administering antipsychotics, the literature devotes sufficient space to their effectiveness on positive symptoms. Cardiovascular and metabolic side effects of antipsychotic medication and appropriate recommendations for their correction are often discussed. Recently, the impact of antipsychotics on negative symptoms has also been amply depicted. The topic of hyperprolactinemia and especially osteoporosis in relation to antipsychotic treatment is less common in the Czech literature. However, it is likely that by improving the treatment of psychoses, patients will live to an older age, and the more they may face the consequences of drug-induced hyperprolactinemia, including osteoporosis. Antipsychotic treatment often causes an increase in serum prolactin, which can lead to hypogonadism. This then directly or indirectly affects bone metabolism. The ability of particular antipsychotics to affect prolactinemia may direct the steps in reduction of the long-term antipsychotic treatment induced osteoporosis risk.

BACKGROUND: Alzheimer's disease (AD) is a progressive neurodegenerative disorder with a complex pathogenesis and a common occurrence of comorbid diseases such as depression. It is accepted that the presence of the ε4 allele of the gene that encodes apolipoprotein E (APOE) is the strongest genetic risk factor for the development of sporadic AD. Melatonin, cortisol, homocysteine, and prolactin are presumed to be risk factors or biomarkers for stress- and age-related disorders. OBJECTIVE: The interplay between the APOE genotype and plasma biomarkers was examined in patients with AD presenting with or without depression to contribute to understanding the interdependence of various molecular mechanisms in the pathophysiology of AD. METHOD: The APOE genotype and morning plasma melatonin, cortisol, homocysteine, and prolactin concentrations were measured in 85 patients with AD and 44 elderly controls. RESULTS: A significant association between AD and the allele (ε4) or genotype (ε3/ε4 or ε4/ε4) frequencies of APOE was confirmed. Plasma homocysteine and cortisol levels were significantly increased in patients with AD compared to those in controls, independent of the presence of comorbid depressive symptoms or the severity of dementia. Significantly lower plasma melatonin concentration was found in patients with AD but not in controls, who were noncarriers of the APOE ε4 allele, regardless of the presence of depression or the severity of dementia in AD. CONCLUSION: Our findings indicate the existence of a little-known specific APOE-mediated mechanism that increases the plasma melatonin level in a subgroup of patients with AD who are carriers of the APOE ε4 allele.

RATIONALE: Self-report studies indicate that cannabis could increase sexual desire in some users. We hypothesized that intoxication increases activation of brain areas responsive to visual erotica, which could be useful in the treatment of hypoactive sexual desire disorder, a condition marked by a lack of sexual desire. OBJECTIVES: The aim of this study is to assess the aphrodisiacal properties of cannabis. METHODS: We conducted an open-randomized study with 21 heterosexual casual cannabis users. A 3T MRI was used to measure brain activation in response to erotic pictures. Blood samples were collected to determine the serum levels of cannabinoids, cortisol and prolactin. Participants were grouped according to whether they had ever experienced any aphrodisiacal effects during intoxication (Group A) or not (Group non-A). RESULTS: Intoxication was found to significantly increase activation in the right nucleus accumbens in the Group A while significantly decreasing activation in the Group non-A. There was also a significant interaction between the group and intoxication, with elevated prolactin in the Group non-A during intoxication. No intoxication-related differences in subjective picture evaluations were found. CONCLUSION: Cannabis intoxication increases activation of the right nucleus accumbens to erotic stimuli. This effect is limited to users whose prolactin is not elevated in response to intoxication. This effect may be useful in the treatment of low sexual desire.

Hyperprolaktinemie je častým laboratorním nálezem a vyskytuje se za fyziologických stavů, u endokrinologických a neendokrinologických onemocnění. Nejčastější příčinou patologické hyperprolaktinemie jsou prolaktinomy, které jsou i nejčastějšími hypofyzárními adenomy. Hyperprolaktinemie se u žen manifestuje infertilitou, poruchami menstruačního cyklu s hypoestrinismem a galaktoreou, u mužů ztrátou libida a potence. Klinicky manifestní hyperprolaktinemii je nutné léčit s ohledem na příčinu hyperprolaktinemie. U prolaktinomů je léčbou volby medikamentózní léčba dopaminergními agonisty. Chirurgická a radiační léčba se uplatňují u rezistentních prolaktinomů, při intoleranci dopaminergních agonistů nebo při komplikacích. V graviditě se u žen s mikroprolaktinomy léčba dopaminergními agonisty přerušuje, u žen s makroprolaktinomy je nutné postupovat individuálně.
Hyperprolactinemia is a common laboratory finding. It is found in some physiological states and in endocrinological and nonendocrinologcal diseases. Prolactinomas are the most common cause of pathologic hyperprolactinemia and the most frequent type of pituitary adenoma. Hyperprolactinemia causes infertility, disturbance of the menstrual cycle due to hypoestrogenism and galactorrhea in women, and decreased libido and impotency in men. In cases of the clinical manifestation of hyperprolactinemia, it is necessary to treat its cause. The primary therapy for prolactinomas is pharmacological treatment with dopamine agonists. Surgical and radiation treatments are used for prolactinoma patients who are resistant or intolerant to dopamine agonists or in the case of complications. During pregnancy of women with microprolactinoma, the pharmacological treatment with dopamine agonists is withdrawn, with pregnant women with macroprolactinoma, it is necessary to assess every single case individually.

BACKGROUND: Celiac disease (CD) is an organ-specific autoimmune disease, and both adaptive and innate immunity are involved in its development. OBJECTIVES: The aim of the study was to determine whether the markers of intestinal mucosal inflammation in CD can be detected in peripheral blood monocytes (PBMs), and whether the immune properties of PBMs change as the clinical signs and symptoms of CD improve after the introduction of a gluten-free diet (GFD). The focus was on changes in mRNA expression of selected toll-like receptors (TLR2, TLR4, TLR7), stress cytokine prolactin (PRL), and proand anti-inflammatory cytokines (TNF-α, IL-6, IL-12, IL-10) in PBMs. MATERIAL AND METHODS: The study involved 20 CD patients diagnosed according to the European Society for Pediatric Gastroenterology, Hepatology and Nutrition criteria and Marsh criteria: 10 recently-diagnosed cases (rCD) and 10 on a GFD for a minimum of one year. The control group comprised 10 ageand sex-matched healthy volunteers. PBMs from peripheral blood specimens were separated using immunomagnetic CD14+ beads. Total RNA was isolated using a standard commercial kit. Cytokine and TLR mRNA levels were quantified by relative qPCR with PGK1 as a reference gene. RESULTS: Significantly higher expression of TLR4 and TLR7 mRNA was observed in PBMs from rCD patients compared to the healthy controls (1.63 times higher; p < 0.05). TLR7 mRNA levels in rCDs were also significantly elevated in comparison to the CD-GFD patients (2.11 times higher; p < 0.01). TNF-α mRNA expression tended to be higher in both groups of patients; by contrast, in IL-6 mRNA, a trend to a fourfold decrease was detected in PBMs from the CD-GFD subjects. IL-10, IL-12 and PRL levels did not differ among the groups. CONCLUSIONS: The data suggest that the inflammatory process in rCD intestinal mucosa and submucosa reflecting enterocyte damage can be detected in PBMs in peripheral blood. Further, the cytokine and TLR expression profile in PBMs alters after one year of GFD treatment.

The aim of our work was to provide data from women of fertile age with type 1 diabetes mellitus about the endogenous androgens and on their relations to the parameters of diabetes control. Forty-two women were examined, they did not use contraceptives for at least three months prior to the examination. A multivariate regression analysis showed that the daily insulin dose, the fasting glycemia and the HbA1c values and patient's age correlated negatively with dehydroepiandrosterone sulfate, dehydroepiandrosterone and prolactin levels. The testosterone/dehydroepiandrosterone sulfate ratio correlated positively with daily insulin dose and patient's age. In contrast to adrenal androgens the values of other hormones, including total and free testosterone, androstenedione, dihydrotestosterone, estradiol, LH, FSH, 17-OH-P, progesterone and cortisol revealed no significant correlation. To conclude, significant relations between the glucose control parameters and the adrenal androgens and prolactin were demonstrated. These relationships should be considered as an important factor influencing diabetes control so the additional cardiovascular risk in women with DM1.
