The article describes the nutmeg of a nutmeg tree (Myristica), which is used as a spice and traditional medicine but is also abused as an illicit drug. The main chemical components, namely myristicin, are described.

Opioidy jsou analgetikem první volby pro středně silnou a intenzivní nádorovou bolest. Jejich role je nezastupitelná. Nádorová bolest je ve většině případů opioid-senzitivní. Nezbytné je precizní monitorování vedlejších účinků. K farmakologickým možnostem v prevenci nežádoucích účinků léčby opioidy patří razantní symptomatická léčba, využití opioid-šetřících režimů a principu rotace. Důležité je včasné nasazení silných opioidů s vynecháním II. stupně analgetického žebříčku. Dostatečný výběr z několika molekul opioidů a jejich lékových forem umožňuje individualizaci terapie, přesto je i v dnešní době nádorová bolest stále léčena nedostatečně.

Opioids are first-choice analgesics for the treatment of moderate to severe cancer pain. Their role cannot be substituted. Most cases of cancer pain are opioid-sensitive. Precise monitoring of side effects is essential. Pharmacology options to prevent opioid side effects include the use of opioid-sparing regimens, dynamic symptomatic treatment, and the use of the principle of opioid rotation. An early start with strong opioids is important, omitting the second step in the analgesic ladder is recommended. Sufficient choice from several opioid molecules and their dosage forms allows individualisation of therapy. Even so, treatment of cancer pain is still insufficient today.

Cílem léčby chronické bolesti opiody je zmírnění bolesti se zlepšením funkčního stavu a zlepšení celkové kvality života pacientů. O úspěšné analgetické léčbě lze hovořit pouze tehdy, pokud úleva od chronické bolesti jednoznačně převyšuje závažnost nežádoucích vedlejších účinků, jejichž nezvládnutí se mnohdy stává bariérou této léčby. Vedlejší nežádoucí účinky opioidů nejsou důvodem k přerušení léčby, ale k řešení těchto reakcí. Velice důležitá je pak prevence vzniku těchto nežádoucích vedlejších účinků léčby (prevence zvracení, obstipace, sedace, delirantních stavů či dechového útlumu).
