Kazuistické sdělení popisuje případ pacientky s adultní formou Pompeho choroby (GSD II). Diagnóza pacientky byla stanovena ve věku 21 let a nemocná je nyní úspěšně léčena substituční terapií. Autorka přehledně shrnuje současný popis znalostí o etiopatogenezi, klasifikaci, diagnostických postupech a léčebných možnostech této vrozené choroby.
A case report is presented of a female patient with adult-onset Pompe disease (GSD II). The patient was diagnosed when 21 years old and is now being successfully treated with replacement therapy. The author provides an overview of the current knowledge on the aetiopathogenesis, classification, diagnostic procedures, and treatment options of this congenital disease. Pompe disease (glycogen storage disease type 2, deficiency of alpha-glucosidase enzyme) is a hereditary autosomal recessive metabolic disorder caused by the deficiency of the lysosomal acid alpha-glucosidase enzyme leading to the accumulation/storing of lysosome glycogen in cells and tissues followed by their dysfunction, especially in the heart and skeletal muscles. The structural gene of alpha-glucosidase has been found on chromosome 17q23. The pathogenesis is variable and dependent on the remaining activity of the alpha-glucosidase. If the cleaving enzyme is almost or completely non-functional, the infantile or classic onset form is usually developed. Symptoms usually develop in the first months of life in the form of general ill-development and muscle weakness, which affects even respiratory muscles, liver and heart. The disease progresses quickly and the death mostly occurs within the first year of age due to cardio-respiratory failure. The late-onset form is characterized by the less progressive phenotype during the emergence of serious mutation of one allele and less severe mutation on the other. The manifestation of this form of the disease occurs from the first decade of life to the sixth decade. Skeletal and respiratory muscles and diaphragm are first to be affected by this form of the disease. The progression is slow and the myocardium usually unaffected. The diagnosis of Pompe disease takes place in two levels. The first level is the screening testing of the dried blood spot (DBS). The diagnosis is then confirmed by determining the activity of acid alpha-glucosidase in white cells. The breakthrough in the field of treatment was the implementation of enzyme replacement therapy (ERT). The drug Myozyme (recombinant human enzyme) is available now. There is a case report of a woman with gradual symptomatic progression from childhood to adulthood in the article. The slowing of the progression and medical problems due to the substitution therapy is also shown.
Pompeho nemoc je vzácné progresivní autosomálně recesivní onemocnění způsobené deficitem lysosomálního enzymu α-glukosidázy (GAA; nebo také kyselé maltázy). Povědomí o tomto onemocnění v odborné veřejnosti vzrostlo ve spojení s příchodem terapie formou podávání náhradního rekombinantního enzymu. Cílem přehledového článku je obeznámit čtenáře s některými méně známými aspekty tohoto zajímavého onemocnění.
Pompe disease is a rare progressive autosomal recessive disorder caused by deficiency of lysosomal α-glucosidase (GAA;or acid maltase). The awareness of the disease among specialists has risen in the association with the arrival of therapy inthe form of applied recombinant enzyme. The aim of this review is to bring the reader some less known aspects of thisinteresting disease.

Pompeho nemoc, označovaná také jako glykogenóza typu II, je vzácné autosomálně recesivně dědičné onemocnění způsobené mutací genu pro enzym označovaný jako kyselá α glukosidáza. Manifestace onemocnění může nastat v jakémkoliv věku. V novorozeneckém a kojeneckém věku se Pompeho nemoc projevuje jako typická výrazná svalová hypotonie, kardiomegalie a respirační insuficience až respirační selhání. V pozdějším dětství a v dospělosti se one-mocnění manifestuje zejména progredující svalovou slabostí a respirační nedostatečností. Diagnostika onemocnění je založena v první fázi na zjištění aktivity enzymu v suché krevní kapce. Potvrzení diagnózy se provádí vyšetřením aktivity α glukosidázy v leukocytech. Vyšetření DNA je důležité pro stanovení korelace mezi genotypem a fenotypem a pro detekci přenašečů v rodině. V současné době dostupná substituční enzymatická terapie výrazně zlepšila klinický stav pacientů i jejich kvalitu života. Článek je zaměřen na patogenezi, klinický obraz, diagnostiku, a především na možnosti léčby tohoto onemocnění.
Pompe disease, or glycogen storage disease type II, is a rare autosomal recessive disorder caused by mutation in gene that encodes the enzyme acid α glucosidase. Clinical manifestation can start in childhood as well as in adulthood. Severe muscle weakness, cardiomyopathy, and respiratory insufficiency or failure are typical in affected infants. In adulthood, the illness presents with progressive muscle weakness and respiratory insufficiency. First step in the diagnostic process consists of a screening using dried blood spot test. The diagnosis in being confirmed by measurement of α glucosidase activity in leukocytes. DNA testing is important for correlation between the genotype and phenotype as well as for detection of carriers within the family. Treatment with recombinant α glucosidase improves the clinical status of these patients and the quality of their lives. This review is focused on the pathogenesis, presentation, diagnostic process, and treatment options.

Lysosomální onemocnění jsou vzácné, dědičně podmíněné nemoci způsobené nedostatečnou aktivitou některého z lysosomálních enzymů, event. transportních proteinů. První příznaky se mohou objevit kdykoli od novorozeneckého období až do pozdní dospělosti, časné formy mívají těžký průběh s rychlou progresí a infaustní prognózou. Postižení je multisystémové s trvalou progresí obtíží a postižením metabolicky aktivních orgánů či tkání – kostní dřeň, játra, kosti, kosterní svaly, myokard, CNS. Diagnózu definitivně potvrdíme průkazem snížené aktivity daného enzymu a mutační analýzou. Některé ze střádavých nemocí můžeme účinně léčit podáním rekombinantních enzymů intravenózně či omezením množství střádaného substrátu. U malého počtu lysosomálních onemocnění je úspěšná transplantace kostní dřeně. Je nutná mezioborová spolupráce včetně zajištění genetického poradenství a prenatální diagnostiky v rodinách pacientů. První část sdělení je věnována obecné charakteristice lysosomálních onemocnění a problematice nejčastějších onemocnění, které je možné v současné době v České republice léčit (Gaucherova nemoc, Pompeho nemoc, Fabryho nemoc, Niemann-Pickova nemoc, nemoc ze střádání esteru cholesterolu). Ve druhé části článku je věnována pozornost problematice mukopolysacharidóz, které představují další skupinu vzácných lysosomálních onemocnění.
Lysosomal storage diseases are rare genetic diseases caused by insufficient activity of some of the lysosomal enzymes and/or transport proteins. Initial symptoms may appear any time from the neonatal period to late adulthood; early forms tend to have a severe course with rapid progression and unfavorable prognosis. There is multisystem involvement with continuous progression of symptoms and involvement of metabolically active organs or tissues – the bone marrow, liver, bones, skeletal muscles, myocardium, or CNS. The diagnosis is definitively confirmed by demonstration of reduced activity of the particular enzyme and by mutation analysis. Some of the storage diseases can be effectively treated by intravenous administration of recombinant enzymes or by limiting the amount of the substrate stored. In a small number of lysosomal storage diseases, bone marrow transplantation is successful. Multidisciplinary collaboration, including genetic counseling and prenatal diagnosis in patient families, is required. The first part of the paper deals with general characteristics of lysosomal storage diseases and the most common diseases that are currently treatable in the Czech Republic (Gaucher’s disease, Pompe disease, Fabry disease, Niemann–Pick disease, cholesterol ester storage disease). The second part of the paper deals with mucopolysaccharidoses, another group of rare lysosomal storage diseases.

Pokroky v léčbě svalových dystrofií lze rozdělit na dvě základní skupiny. Aktuální symptomatickou péči a pokroky v genové a patogeneticky orientované terapii. Symptomatická péče, přestože neléčí podstatu nemoci, je velmi významná a zvyšuje kvalitu a délku života nemocných. Jde o prodlužování schopnosti chůze pomocí ortéz, podporu ventilace a kardiologickou péči. Srdečnímu postižení je třeba věnovat odpovídající pozornost, u některých svalových chorob jde o limitující postižení (Emeryho Dreifussova svalová dystrofie, dystrofinopatie). Za symptomatickou farmakoterapii lze také považovat podávání kortikosteroidů u chlapců s dystrofinopatií. Etiopatogeneticky orientovaná léčba se u pacientů s dystrofinopatií dělí na genovou terapii, buněčnou terapii a farmakoterapii. Experimenty v oblasti genové terapie se soustředí na přenos mikrodystrofinu do genomu pomocí virového nosiče. Hlavní překážkou je zatím autoimunitní reakce organizmu. Se stejnými potížemi se potýká i buněčná terapie s lokální aplikací myoblastů, mezangioblastů a CD133+ buněk. Farmakoterapie je zaměřena na léky podporující upregulaci exprese utrofinu, který může částečně nahradit funkci dystrofinu: bylo zahájeno testování látky BMN195 na zdravých dobrovolnících. Dalším perspektivním směrem jsou inhibitory myostatinu, tedy látky zasahující do funkce genu GDF-8, jde o monoklonální protilátky ACE-031 a MYO-029 a látku ACVR2B. Další velkou skupinou jsou tzv. mutačně specifické přístupy. Látka PTC 124 umožňuje u nemocných s dystrofinopatií Duchennerova typu překlenutí bodové mutace se stop kodonem a tedy tvorbu sice mutovaného, ale alespoň částečně funkčního dystrofinu. Nadějné experimentální výsledky se bohužel neukázaly v klinické praxi účinné a klinická studie ve fázi IIb byla zastavena (Ataluren®). Největší pokrok dosáhla zatím strategie, která se nazývá „exon skipping“, tedy přeskočení deletovaných exonů u nemocných s Duchenneovou svalovou dystofiií, což umožňují tzv. antisense oligonukleotidy (AON), tedy speciální malé části RNA. Výsledkem je opět produkce alespoň částečně funkčního dystrofinu. Na každý hot spot je třeba připravit jiný AON. Nejdále je v klinickém výzkumu lék firmy Prosensa, což je AON sloužící k přemostění exonu 51. V roce 2010 byla s tímto lékem zahájena multicentrická mezinárodní klinická studie. U myotonické dystrofie se potenciální léčebné strategie soustředí na klíčový pochod, kterým je hromadění expandované RNA a její toxické účinky na intracelulární pochody.
Treatment progress in hereditary neuromuscular disorders could be divided in two groups. Current multidisciplinary care and progress in gene and pathogetic oriented therapy. Multidisciplinary symptomatic care does not treat the principle of the disease, but it is very significant and improves the patient's quality of life. Orthoses which can prolong the ambulatory stages, management of cardiac and pulmonary functions are considerable topics. Cardiac involvement is in some muscular dystrophies the prominent sign which can restrict the life length expectancy (Emery- Dreifuss muscular dystrophy, dystrophinopathy). To the symptomatic care belongs also the administration of corticosteroids in boys with Duchenne muscular dystrophy. Causative treatment in patients with Duchenne muscular dystrophy has many ways: gene therapy, cell therapy, and drug therapy. Gene therapy is focused to the transfer of the micro- dystrophin using viral vectors. The main problem of this way is the immune response of the human muscle tissue. The same problem is generated by local transfer of the myoblasts, mesangioblasts, and CD133+ cells. Drug therapy is dealing with upregulation of utrophin which can partially replace the dystrophin; BMN-195 compound was tested in healthy volunteers. Different possible therapeutic way are the inhibitors of myostatin: compounds which can influence the gene GDF-8: monoclonal antibodies ACE-031 and MYO-029, and compound ACVR2B. Mutation-specific approaches are the methods based on the correction of the genetic code, which allows the production of a partially functional dystrophin. Promising experimental results with PTC124 showed no functional improvement in human volunteers. The most promising results was reached with an „exon skipping“ therapy, which can restore the genetic code from Duchenne patients. A partially functional, Becker-like dystrophin protein can be produced instead of a non functional Duchenne protein. This is achieved by AONs (antisense oligonucleotides). AONs are small pieces of modified RNA. Different AONs are needed for each hot spot. Drug named PRO051 (AON for skipping of deleted exon 51) is currently tested and during 2010 multi centric international clinical study Phase III has been started. In patients with myotonic dystrophy type I the therapeutic strategies are focused on expanded RNA and its toxicity on intracellular processes.
