Mirtazapin patří mezi antidepresiva s duálním účinkem (NaSSA), ovlivňuje serotonergní a noradrenergní neurotransmisi. Jedná se o antidepresivum s vysokou účinností v léčbě deprese – srovnatelnou nebo vyšší než u ostatních antidepresiv. Dále se od ostatních antidepresiv liší nástupem účinku a profilem nežádoucích účinků. Cílem článku je podat bližší informace o mirtazapinu a ukázat, pro jaký typ pacientů je vhodnou léčebnou volbou.
Mirtazapine belongs to antidepressants with a dual mechanism of action (NaSSAs), influencing serotonergic and noradrenergic neurotransmission. It is an antidepressant with a high efficacy in the treatment of depression. The efficacy of mirtazapine is comparable with or higher than that of other antidepressants. It is different in the onset of action and in the profile of adverse events. The aim of this article is to provide more information about this antidepressant and describe the type of patients who are suitable for this medication.

OBJECTIVE: Our study aimed to investigate the effect of mirtazapine on bone metabolism in the orchidectomized rat model. METHODS: Rats were divided into three groups. A sham-operated control group (SHAM group) and a control group after orchidectomy (ORX group) received the standard laboratory diet (SLD). An experimental group after orchidectomy (ORX MIRTA group) received SLD enriched with mirtazapine for 12 weeks. Bone mineral density (BMD) was measured by dual-energy X-ray absorptiometry. Bone marker concentrations of osteoprotegerin (OPG), amino-terminal propeptide of procollagen type I, bone alkaline phosphatase (BALP), sclerostin and bone morphogenetic protein 2 were examined in bone homogenate. The femurs were used for biomechanical testing. RESULTS: Compared with the control ORX group, we found a lower BMD in the ORX MIRTA group. The differences were statistically significant, although not in the lumbar vertebrae. BMD was lower in the MIRTA group, suggesting a preferential effect on cortical bone. However, although the thickness of the diaphyseal cortical bone was not different, the fragility in the femoral neck area was statistically significantly different between the groups in biomechanical testing. Regarding the bone metabolism markers, there was a significant decrease in OPG and BALP levels, suggesting a reduction in osteoid synthesis. CONCLUSIONS: The results suggest that prolonged use of mirtazapine may have a negative effect on the synthesis of bone and on its mechanical strength, especially in the femoral neck. Further studies are warranted to establish whether mirtazapine may have a clinically significant adverse effect on bone exclusively in the model of gonadectomized rats, or whether the effect occurs also in the model of gonadally intact animals and in respective human models.

Cíl práce: Kazuistika pacientky s močovou inkontinencí indukovanou antidepresivem mirtazapine a přehled literatury k tématu. Typ práce: Kazuistika. Název a sídlo pracoviště: Urologické oddělení FN Ostrava-Poruba. Vlastní pozorování: Prezentujeme případ 55leté pacientky, která byla odeslána k operačnímu řešení inkontinence moči. V průběhu vyšetření byly zjištěny zásadní diskrepance, které nás vedly k podezření, že nejde o běžnou nekomplikovanou stresovou inkontinenci. Díky podrobnému rozboru anamnézy a časových sousledností byl identifikován jako pravděpodobný vyvolávající činitel mirtazapin. Po jeho vysazení pacientka dosáhla plné kontroly kontinence. Závěr: Vzhledem k tomu, že antidepresiva ovlivňují adrenergní a dopaminergní regulační mechanismy v centrálním nervovém systému, mohou mít vliv na funkci dolních močových cest. Práce prezentuje kazuistiku, kde odhalení méně časté příčiny inkontinence ušetřilo pacientku původně plánované operace a umožnilo efektivní sanaci inkontinence. Zdůrazněna je nutnost zvažovat potenciální interakce antidepresiv s funkcí dolních močových cest v denní praxi.
Aim: Case study of the patient with urinary incontinence induced by the antidepressant mirtazapin and the review of the related literature. Design: Case report. Setting: Department of Urology, University Hospital Ostrava-Poruba. Case report: A case of 55-years old patiens, who was reffered to the surgical treatment of the urinary incontinence. We found a major discrepancies during the evaluation that led us to suspect that this is not a common uncomplicated case of stress urinary incontinence. Based on the detailed history we identified the antidepressant mirtazapine as a likely causal factor. After discontinuing mirtazapin patient has achieved full control of the continence. Conclusion: Given that antidepressants affects adrenergic and dopaminergic regulatory mechanisms in the central nervous system, they may affect the lower urinary tract function. This work presents a case report where the disclosure of the less common cause of incontinence saved the patientoriginally proposed surgical treatment and allowed the effective restoration of the continence. We emphasize the need to consider the potential interaction of antidepressants with lower urinary tract function in daily practice.

Poruchy spánku tvoří velmi nesourodou skupinu onemocnění. V psychiatrické ordinaci se setkáváme nejčastěji s primární a sekundární nespavostí, kde nespavost je jeden z příznaků jiného nemocnění. V léčbě chronické nespavosti můžeme použít některá tricyklická antidepresiva, trazodon, mirtazapin a melatonin. Léky mají v léčbě primární nespavosti většinou pomocný význam. Nejefektivnější je ovlivnit chování, které se váže ke spánku, pomocí psychoterapie. V současné době se považuje za nejpropracovanější a nejefektivnější kognitivně behaviorální terapie.
Sleep disorders comprise a very heterogeneous group of diseases. In psychiatric practice, we most commonly encounter primary and secondary insomnia, wherein insomnia is a symptom of another disease. Some tricyclic antidepressants, trazodone, mirtazapine, and melatonin can be used in treating chronic insomnia. Drugs generally are of additional importance in treating primary insomnia. The most effective approach is to affect the behavior that is linked to sleep by means of psychotherapy. Currently, cognitive behavioral therapy is considered to be the most sophisticated and effective method.

OBJECTIVE: Monoamine oxidase (MAO), the enzyme responsible for metabolism of monoamine neurotransmitters, has an important role in the brain development and function, and MAO inhibitors have a range of potential therapeutic uses. We investigated systematically in vitro effects of pharmacologically different antidepressants and mood stabilizers on MAO activity. Methods: Effects of drugs on the activity of MAO were measured in crude mitochondrial fraction isolated from cortex of pig brain, when radiolabeled serotonin (for MAO-A) or phenylethylamine (for MAO-B) was used as substrate. The several antidepressants and mood stabilizers were compared with effects of well known MAO inhibitors such as moclobemide, iproniazid, pargyline, and clorgyline. Results: In general, the effect of tested drugs was found to be inhibitory. The half maximal inhibitory concentration, parameters of enzyme kinetic, and mechanism of inhibition were determined. MAO-A was inhibited by the following drugs: pargyline > clorgyline > iproniazid > fluoxetine > desipramine > amitriptyline > imipramine > citalopram > venlafaxine > reboxetine > olanzapine > mirtazapine > tianeptine > moclobemide, cocaine > lithium, valproate. MAO-B was inhibited by the following drugs: pargyline > clorgyline > iproniazid > fluoxetine > venlafaxine > amitriptyline > olanzapine > citalopram > desipramine > reboxetine > imipramine > tianeptine > mirtazapine, cocaine > moclobemide, lithium, valproate. The mechanism of inhibition of MAOs by several antidepressants was found various. Conclusions: It was concluded that MAO activity is acutely affected by pharmacologically different antidepressants at relatively high drug concentrations; this effect is inhibitory. There are differences both in inhibitory potency and in mechanism of inhibition between both several drugs and the two MAO isoforms. While MAO inhibition is not primary biochemical effect related to their therapeutic action, it can be supposed that decrease of MAO activity may be concerned in some effects of these drugs on serotonergic, noradrenergic, and dopaminergic neurotransmission.

OBJECTIVE: Mirtazapine is a new antidepressant used in last years, however experience with it during pregnancy is unsatisfactory on the present. Its wide therapeutic range and only little proved side effects may be an advantage for treatment during pregnancy. Aim of our study was to contribute to the knowledge on possible risks. MATERIALS AND METHODS: For embryotoxicity testing we used an alternative method - CHEST, that used chicken embryos as experimental model. Fertilized eggs of outbred Grey Leghorn stock (AVČR farm Koleč) were treated on embryonic day (ED) 4 by Mirtazapine, incubated till 9ED, when they were weighed and examined. Summing the proportions of dead and malformed embryos, the beginning of the embryotoxicity dose range was estimated. RESULTS: Mirtazapine solved in 15% DMSO in water revealed low embryotoxicity corresponding data from preclinical studies. If 100% DMSO was used as a solvent, the dose 0.05 μg/3 μL resulted in 57% mortality (LD50). Typical malformations were microphtalmia and malformation (shortening) of limbs on left side, which is a place of contact the embryonic body with maximal Mirtazapine concentration. Approximation of doses in chick embryos to mammals is complicated by low solubility of mirtazapine. CONCLUSIONS: If the embryotoxic dose was close to LD50, risk at therapeutical doses will be probably low. Mirtazapine according to results of testing and cases published in literature is relatively safe for pregnant women, only higher rate of abortions was demonstrated, however more information is needed to exclude all potential risks.
