Fine needle aspiration cytology (FNAC) is an important and primary screening tool for all breast lumps with high level of sensitivity and specificity. Diagnosing the rare breast carcinoma on FNAC is very challenging on account of various mimickers of the same. We hereby describe a rare case of breast carcinoma diagnosed on FNAC in a 34-year-old female confirmed on histopathology and immunohistochemistry (IHC). Preoperative diagnosis of apocrine carcinoma on FNAC and later confirmation on histopathology along with IHC study gives better insight to the cytopathologist and helps in better management protocol of the patient.

This longitudinal study was aimed to evaluate the electromyographic activity (EMG) of the masticatory muscles during mandibular tasks in women with grade II and III obesity, who were eligible for bariatric surgery. Twenty-one patients were followed up for 3 and 6 months after the Roux-en-Y gastric bypass. The EMG included analyses of the masseter and temporalis muscles during rest, right and left laterality, protrusion, and dental clenching at maximal voluntary contraction with and without parafilm. Data were tabulated and submitted for statistical analysis using the repeated measures test (P<0.05) with the Bonferroni post-test. Significant differences were observed between the right temporal muscle during maximal voluntary contraction (P=0.003) and maximal voluntary contraction with Parafilm M for the right masseter (P=0.01), left masseter (P=0.03), right temporal (P=0.002), and left temporal (P=0.03) muscles. There was gradual decrease in the resting EMG of the masticatory muscles 6 months after surgery. There was an increase in the EMG of the muscles that are the most active in the neuroanatomical movements of laterality and protrusion 6 months after surgery. There was an increase in EMG in maximal voluntary contraction with and without parafilm, 3 and 6 months after surgery. This study suggests that women with severe obesity who underwent bariatric surgery had better functional results of the masticatory muscles 3 and 6 months after the procedure, which is an important parameter in surgical planning, functional recovery, and nutritional status.

The COVID-19 pandemic significantly impacted educational methods worldwide, particularly in dental and general medicine education. This study retrospectively analyzes students' perceptions of hybrid theoretical and practical teaching in the Dentistry program during the pandemic at three medical faculties of Charles University in the Czech Republic. A total of 418 students were surveyed regarding their views on hybrid education, concerns about COVID-19, and the pandemic's psychosocial and academic effects. The study revealed notable differences based on faculty location, gender, year of study, language of instruction, and the phase of study affected. Students from smaller faculties (Hradec Kralove and Pilsen) preferred online education and did not report a significant reduction in lectures. Compensatory block teaching post-pandemic was effective but less preferred, especially among women. Men found online communication more challenging than women. Czech-speaking students rated online teaching more effective than those studying in English and compensated for clinical skill gaps by participating in more internships. Future online education methods should address the unique needs of these groups.

Fluid collections are common manifestations of local complications of acute pancreatitis (AP). The determination of fluid collections is important in stratifying the severity of the course of acute pancreatitis and choosing the appropriate management. In this study, the frequency of detection of fluid effusions by various instrumental methods and their clinical significance in 677 patient was assessed, taking into account the severity of the course of AP and anatomical localization of fluid accumulations. The computed tomography (CT) method showed its advantage in detecting fluid effusions in the pleural cavities, while the ultrasound method showed its indisputable significant advantage in the diagnosis of fluid effusions located parapancreatically and in the lesser sac: the highest frequency of detection of fluid effusion was in 65.3% of all patients in the general group using CT compared to 39% using ultrasound and 12.5% using chest X-ray (p<0.05). Each of the above methods showed different sensitivity depending on the anatomical areas of fluid effusion detection. In particular, the CT method revealed the presence of fluid in the pleural cavities in 50.7% of the total group, in the abdominal cavity in 36.0% of cases, and only in 8.0% of cases in the parapancreatic or lesser sac. The opposite picture was revealed when using the ultrasound method, in which fluid was found in the parapancreatic/lesser sac in the largest number of cases - 28.1%, in the abdominal cavity - in 22.6% of cases, and in the pleural cavity - in 14.3% of cases.

Referrals to emergency services are frequently made for acute appendicitis and renal colic. This conundrum of patients with acute appendicitis and right ureteral stones frequently presents itself to emergency physicians. The hematological parameters of patients suffering from acute appendicitis and right ureteral stones were compared in this study. From May 2022 to April 2023 the patients who applied to emergency department were reviewed retrospectively. Of these patients, age, sex, complete blood test results including white blood cell, procalcitonin and C-reactive protein were recorded. The platelet to lymphocyte ratio and the neutrophil to lymphocyte ratio were calculated by dividing the platelet count by the lymphocyte, and the neutrophil count by the lymphocyte count, respectively. We used the receiver operating characteristic curves to assess the diagnostic efficacy of hematological markers. There were 106 patients in the study. The mean age of the patients was 43.83 ± 15.82 years. The significant difference was found for age, white blood cell, neutrophil, lymphocyte, neutrophil to lymphocyte ratio, platelet to lymphocyte ratio and monocyte between groups. The white blood cell and neutrophil to lymphocyte ratio have the biggest area than the other parameters in the receiver operating characteristic curves. Patients with acute appendicitis have a high level of neutrophil to lymphocyte ratio, platelet to lymphocyte ratio than ureteral stones. Further studies are needed to define the diagnostic efficacy of hematological markers for acute appendicitis and ureteral stones.

We have verified the eligibility of the Idiopathic Hypersomnia Severity Scale (IHSS) as a basic clinical tool for determining the subjective severity of illness in patients with idiopathic hypersomnia (IH) in the Czech Republic. Total of 37 patients with a diagnosis of IH (9 men, 28 women, mean age 40.2 ± 12.8) completed the IHSS scale. At the same time, they were instructed to complete the Epworth Sleepiness Scale (ESS), the Fatigue Severity Scale (FSS), the Hospital Anxiety and Depression Scales (HADS-A and HADS-D), and a short version of the Quality of Life Questionnaire (SF-36). The control group consisted of 88 age- and sex-matched healthy volunteers. The IHSS scale showed good internal consistency of the questionnaire using Cronbach's α, which was 0.88. The KMO (Keiser-Meyer-Olkin index) was 0.72, confirming sufficient structural validity of the questionnaire. The correlation of the total IHSS score with the ESS (ρ = 0.59, p=0.0001) and FSS (ρ = 0.84, p<0.0001) as well as with the HADS-A scales (ρ = 0.64, p<0.0001), HADS-D (ρ = 0.79, p<0.0001) and SF-36 in both the mental (ρ = -0.85, p<0.0001) and physical health (ρ = -0.66, p<0.0001) components. The IHSS is a convenient and easy-to-apply clinical tool to assess subjective severity of illness, which describes well the symptoms of idiopathic hypersomnia and assesses their impact on health and daily activities.
