INTRODUCTION: The use of signal dogs for cancer detection is not yet routinely performed,but dogs and their powerful olfactory system have proven to be a unique and valuable tool for many lineages and are beginning to be incorporated into medical practice. This method has great advantages; the dog can detect a tumour in the human body already in preclinical stages, when the patient has no symptoms yet. The identification of cancer biomarkers to enable early diagnosis is a need for many types of cancer, whose prognosis is strongly dependent on the stage of the disease. However, this method also has its various pitfalls that must be taken into account. AIM: The aim of the study was to identify and highlight the factors that affect the level of detection accuracy, but also the conditions associated with olfactometric diagnosis. METHODS: The study included 48 dogs and 48 handlers, that were part of the training between 2016 and 2023.All those who started olfactometry training and remained in training for at least one year were included in the study. The dogs ranged in age from 8 months to 12 years and were of different races and sexes. After long-term observation, a qualitative analysis was performed and factors that may play a role in the early detection of the disease were listed. RESULTS: The results of the search for the different factors have been compiled into two groups, focussing on the actual handling of the patient biological sample from collection, processing, storage until transport, preparation of the sample,and detection. Focus on the actual work and behaviour of the dog and handler. CONCLUSION: There are many factors; however, it is worth addressing them because the canine sense of smell is one of the possible uses as a diagnostic method.

Previous reports provided recommendations for familial renal glucosuria diagnosis without complex view on differential diagnosis of glucosuria. The aim of this review was to provide an overview of the causes of glucosuria and to create an evidence-based diagnostic approach for children with glucosuria. We searched the current literature with a focus to identify the possible etiology of glucosuria, gaining insight into the pathophysiology of glucosuria. Urinary glucose is completely reabsorbed in the proximal tubule of kidneys. It only appears in the urine if the plasma glucose concentration exceeds the renal threshold for glucose or in the case of insufficient renal glucose reabsorption. The proteins that provide glucose reabsorption are SGLT2 and SGLT1 - sodium-dependent co-transporters that transport glucose from the lumen into epithelial cells - and GLUT2 - a passive transporter providing facilitative glucose transport from epithelial cells to plasma. Renal glucose reabsorption is affected in case of acquired or inherited complex dysfunction of proximal tubule called Fanconi Syndrome or due to pathogenic variants of genes encoding glucose transporters. Prior to diagnosing any of these, diabetes mellitus must be excluded together with other conditions leading to hyperglycemia. In conclusion, glucosuria is always an abnormal finding. The review provides a simple evidence-based diagnostic approach to navigate the differential diagnosis of glucosuria.

The sacroiliac joint (SIJ) exhibits significant variation in auricular surface morphology. This variation influences the mechanics of the SIJ, a central node for transmitting mechanical energy from upper body to lower limbs and vice versa. The impact of the auricular surface morphology on stress and deformation in the SIJ remains poorly understood to date. Computed tomography scans obtained from 281 individuals were included to extract the geometry of the pelvic ring. Then, the auricular surface area, SIJ cartilage thickness, and total SIJ cartilage volume were identified. Based on these reconstructions, 281 finite element models were created to simulate SIJ mechanical loading. It was found that SIJ cartilage thickness only weakly depended on age or laterality, while being strongly sex sensitive. Auricular surface area and SIJ cartilage volume depended weakly and non-linearly on age, peaking around menopause in females, but without significant laterality effect. Larger SIJs, characterized by greater auricular area and cartilage volume, exhibited reduced stress and deformation under loading. These findings highlight the significant role of SIJ morphology in its biomechanical response, suggesting a potential link between morphological variations and the risk of SIJ dysfunction. Understanding this relationship could improve diagnosis and targeted treatment strategies for SIJ-related conditions.

Cíl: Úraz elektrickým proudem z vysokého napětí je jedním z nejzávažnějších úrazů, se kterými se můžeme v moderní medicíně setkat. Často bývá spojený s více následky a vysokou náchylností k infekčním komplikacím. Tito pacienti jsou přijímáni do specializovaných popáleninových center a vyžadují rozsáhlou multidisciplinární spolupráci. V této studii se snažíme odhalit prevalenci, typy a charakteristiky mikrobiálních infekcí, které se vyvíjejí po vysokonapěťovém elektrotraumatu, a identifikovat rizikové faktory, které mohou přispívat k náchylnosti pacientů k infekcím. Materiál a metodika: Pro účely této publikace byla zpracována data všech 37 pacientů hospitalizovaných na jednotce intenzivní péče Kliniky popálenin a plastické chirurgie FN Brno s diagnózou úraz elektrickým proudem vysokým napětím v letech 2006–2016. Otisky a stěry z exfoliovaných oblastí byly opakovaně odebírány k mikrobiální analýze spolu s tracheobronchiálním aspirátem, sputem nebo bronchoalveolární laváží, močí a periferní krví. Získaná data byla zpětně analyzována. Výsledky: Mezi 37 pacienty byl medián věku 31,9 s průměrnou dobou hospitalizace 44,3 dne a úmrtností 8,1 %. Na umělé plicní ventilaci bylo závislých celkem 28 osob. Výskyt infekčních komplikací se v průběhu hospitalizace liší podle místa kultivace odběru a doby strávené v nemocnici. U 97,3 % pacientů se vyvinula infekce alespoň v jednom tělesném kompartmentu. V 88,8 % případů byla multipatogenní a ve 41,6 % se rozvinul septický stav. V naší studijní kohortě dominovaly G+ nad G-kmeny. Nejčastějšími zástupci z G+ spektra byli koaguláza negativní stafylokoky (97 %), Staphylococcus aureus (57 %), Enterococcus fecalis et faecium (51 %). V G-spektru bylo pořadí následující: Klebsiella pneumoniae (46 %), Pseudomonas aeruginosa (41 %), Escherichia coli (35 %) a Acinetobacter baumannii (18,9 %). Nejčastější pozorovanou infekcí byla infekce popálenin (BWI), následovaná infekcemi krevního řečiště (BSI), infekcemi dolních cest dýchacích (LRTI) a infekcemi močových cest (UTI), primárně způsobené G+ patogeny. Je pozoruhodné, že delší doba hospitalizace byla spojena s rostoucí prevalencí G-patogenů, zejména K. pneumoniae, P. aeruginosa a A. baumannii, které vykazovaly vysoký stupeň antimikrobiální rezistence. Závěr: Tato studie poskytuje podrobný pohled na výskyt a následky úrazů elektrickým proudem s vysokým napětím na Moravě v průběhu desetiletí. Faktory významně ovlivňující přežití a závažnost výsledků zahrnovaly celkovou plochu popálenin, popáleniny v celé tloušťce, inhalační poranění a potřebu tracheostomie. Studie je však limitována relativně malou velikostí vzorku, dlouhou dobou sběru dat s potenciálními změnami v klinické praxi a jednocentrovým designem, což může ovlivnit zobecnění nálezů. K ověření těchto výsledků a zpřesnění strategií prevence infekcí u této populace pacientů jsou zapotřebí další multicentrické studie.
Background and Aim: High voltage electrotrauma is one of the most serious injuries we can encounter in modern medicine, often associated with multiple disabilities and high susceptibility to infectious complications. These patients are admitted to specialized burn centers and require extensive multidisciplinary collaboration. In this study, we aim to uncover the prevalence, types and characteristics of microbial infections that develop in the aftermath of high voltage electrotrauma and to identify risk factors that may contribute to patients’ susceptibility to infections. Material and Methods: For the purposes of this publication, data of all 37 patients hospitalized in the intensive care unit of the Department of Burns and Plastic Surgery of the University Hospital in Brno with a diagnosis of high-voltage electrical injury between 2006–2016 were processed. Imprints and swaps from exfoliated areas were repeatedly taken for microbial analysis, together with tracheobronchial aspirate fluid, sputum, or bronchoalveolar lavage, urine and peripheral blood. The obtained data were analysed retrospectively. Results: Among the 37 patients, the median age was 31.9, with an average hospital stay of 44.3 days and a mortality rate of 8.1%. A total of 28 individuals were dependent on artificial lung ventilation. The incidence of infectious complications varies during the hospitalization period according to the location of sampling cultivation and time spent at the hospital. 97.3% of patients developed infection in at least one body compartment. In 88.8% of cases, it was multipathogenic and in 41.6% a septic condition developed. In our study cohort, G+ dominated over Gstrains. Most common representatives from G+ spectrum were Coagulase negative Staphylococci (97%), Staphylococcus aureus (57%), Enterococcus fecalis et faecium (51%). In Gspectrum, the order was as followed: Klebsiella pneumoniae (46%), Pseudomonas aeruginosa (41%), Escherichia coli (35%) and Acinetobacter baumannii (18.9%). The most common infection observed was burn wound infection (BWI), followed by bloodstream infections (BSI), lower respiratory tract infections (LRTI), and urinary tract infections (UTI), primarily caused by G+ pathogens. Notably, an increased hospital stay duration was associated with a rising prevalence of Gpathogens, particularly K. pneumoniae P. aeruginosa and A. baumannii which exhibited a high degree of antimicrobial resistance. Conclusion: This study provides a detailed insight into the occurrence and consequences of high-voltage electrical injuries in Moravia over a decade. Factors significantly impacting survival and severity of outcomes included total burn surface area, full-thickness burns, inhalation injury, and the need for tracheostomy. However, the study is limited by its relatively small sample size, long data collection period with potential changes in clinical practice, and single-center design, which may affect the generalizability of the findings. Further multicentric studies are needed to validate these results and refine infection prevention strategies in this patient population.
BACKGROUND: Preservation of mobility independence is a primary goal in older adults with physical frailty and sarcopenia (PF&S). Interventions based on the combination of physical activity (PA) and nutritional counselling have been indicated as strategies for the management of this condition, although their effectiveness is not confirmed in all investigations. A possible explanation for this uncertain scenario relies in the impact of the adherence to PA interventions. Hence, the present study investigated the impact of the adherence to PA sessions on the incidence of mobility disability in older adults with PF&S. METHODS: This is a secondary analysis of an evaluator blinded, randomised controlled trial, developed in 16 clinical sites across 11 European countries, from January 2016 to 31 October 2019. Participants were community-dwelling older adults (70+ years) with PF&S enrolled in the SPRINTT trial (NCT02582138). PF&S was operationalised as having a total score from 3 to 9 on the short physical performance battery (SPPB), low appendicular lean mass and ability to complete the 400-m walk test in < 15 min. Data from participants allocated to a multicomponent intervention (PA with technological support plus nutritional counselling) and a healthy ageing lifestyle education programme (control group) were analysed. Adherence to PA was assessed based on the number of weekly sessions attended. According to recommendations of the American College of Sports Medicine, adherence was categorised as below recommendations (< 2 sessions/week, BR), meeting recommendations (2-3 sessions/week, MR), and above recommendations (> 3 sessions/week, AR). The primary outcome was incident mobility disability, operationalised as incident inability to complete the 400-m walk test in < 15 min during up to 36 months of follow-up. RESULTS: Data of 1444 participants (mean age 79.3 years, 72.6% women) were analysed. In those with SPPB scores of 3-7, MR and AR groups had lower risk of mobility disability compared with controls [MR HR (95% CI): 0.57 (0.41-0.78), p = 0.001; AR HR (95% CI): 0.33 (0.23-0.46), p < 0.001] and BR groups [MR: HR (95% CI): 0.48 (0.34-0.69), p < 0.001; AR: HR (95% CI): 0.27 (0.18-0.38), p < 0.001] in a dose-dependent manner. In those with SPPB scores of 8 or 9, the BR group had a higher risk of mobility disability than controls. MR and AR groups had a lower risk of mobility disability than the BR group. CONCLUSIONS: In older adults with PF&S, adherence to PA recommendations is associated with lower incidence of mobility disability. This benefit depends on the degree of adherence as well as baseline physical performance. TRIAL REGISTRATION: ClinicalTrials.gov NCT02582138.

3-methylglutaconic aciduria (3-MGCA) is a biochemical finding in a diverse group of inherited metabolic disorders. Conditions manifesting 3-MGCA are classified into two major categories, primary and secondary. Primary 3-MGCAs involve two inherited enzymatic deficiencies affecting leucine catabolism, whereas secondary 3-MGCAs comprise a larger heterogeneous group of conditions that have in common compromised mitochondrial energy metabolism. Here, we report 3-MGCA in two siblings presenting with sensorineural hearing loss and neurological abnormalities associated with a novel, homozygous missense variant (c.1999C>G, p.Leu667Val) in the YME1L1 gene which encodes a mitochondrial ATP-dependent metalloprotease. We show that the identified variant results in compromised YME1L1 function, as evidenced by abnormal proteolytic processing of substrate proteins, such as OPA1 and PRELID1. Consistent with the aberrant processing of the mitochondrial fusion protein OPA1, we demonstrate enhanced mitochondrial fission and fragmentation of the mitochondrial network in patient-derived fibroblasts. Furthermore, our results indicate that YME1L1L667V is associated with attenuated activity of rate-limiting Krebs cycle enzymes and reduced mitochondrial respiration, which may explain the build-up of 3-methylglutaconic and 3-methylglutaric acid due to the diversion of acetyl-CoA, not efficiently processed in the Krebs cycle, towards the formation of 3-methylglutaconyl-CoA, the precursor of these metabolites. In summary, our findings classify YME1L1 deficiency as a new type of secondary 3-MGCA, thus expanding the genetic landscape and facilitating the diagnosis of inherited metabolic disorders featuring this biochemical phenotype.

Obezita a diabetes mellitus 2. typu (DM2T) sú významnými rizikovými faktormi rozvoja kognitívnej dysfunkcie a neurodegeneratívnych ochorení. Ich spoločný patofyziologický základ zahŕňa inzulínovú rezistenciu, chronický subklinický systémový zápal a neurozápal, poruchy mikrobiómu, hormonálnu dysreguláciu a štrukturálne zmeny mozgu. Tieto faktory vedú k zhoršeniu pamäte, exekutívnych funkcií a k akcelerácii neurodegenerácie. Pozitívne účinky úpravy životného štýlu – vrátane zníženia telesnej hmotnosti, zvýšenia fyzickej aktivity a úpravy výživy a stravovacích návykov – sa prejavujú zlepšením inzulínovej senzitivity v mozgu, zvýšením neurotrofických faktorov, redukciou systémového zápalu a neurozápalu a zlepšením metabolizmu. Kombinácia behaviorálnych a farmakologických intervencií môže spomaliť kognitívny pokles a znížiť riziko demencie u populácie s obezitou a poruchou metabolizmu glukózy.
Obesity and type 2 diabetes (T2D) are important risk factors for the development of cognitive dysfunction and neurodegenerative diseases. Their common pathophysiological substrate includes insulin resistance, chronic subclinical systemic inflammation, neuroinflammation, shifts in the intestinal microbiome composition, hormonal dysregulation, and structural changes of the brain. These factors lead to impaired memory, executive functions, and accelerated neurodegeneration. The positive effects of lifestyle modifications — including weight loss, increased physical activity, and improved dietary composition — are manifested by improved insulin sensitivity in the brain, increased neurotrophic factors, reduced systemic inflammation and neuroinflammation, and improved metabolism. A combination of behavioral and pharmacological interventions may slow cognitive decline and reduce the risk of dementia in patients with obesity, prediabetes and T2D.

With the incorporation of effective therapies for myelofibrosis (MF), accurately predicting outcomes after allogeneic hematopoietic cell transplantation (allo-HCT) is crucial for determining the optimal timing for this procedure. Using data from 5183 patients with MF who underwent first allo-HCT between 2005 and 2020 at European Society for Blood and Marrow Transplantation centers, we examined different machine learning (ML) models to predict overall survival after transplant. The cohort was divided into a training set (75%) and a test set (25%) for model validation. A random survival forests (RSF) model was developed based on 10 variables: patient age, comorbidity index, performance status, blood blasts, hemoglobin, leukocytes, platelets, donor type, conditioning intensity, and graft-versus-host disease prophylaxis. Its performance was compared with a 4-level Cox regression-based score and other ML-based models derived from the same data set, and with the Center for International Blood and Marrow Transplant Research score. The RSF outperformed all comparators, achieving better concordance indices across both primary and postessential thrombocythemia/polycythemia vera MF subgroups. The robustness and generalizability of the RSF model was confirmed by Akaike information criterion and time-dependent receiver operating characteristic area under the curve metrics in both sets. Although all models were prognostic for nonrelapse mortality, the RSF provided better curve separation, effectively identifying a high-risk group comprising 25% of patients. In conclusion, ML enhances risk stratification in patients with MF undergoing allo-HCT, paving the way for personalized medicine. A web application (https://gemfin.click/ebmt) based on the RSF model offers a practical tool to identify patients at high risk for poor transplantation outcomes, supporting informed treatment decisions and advancing individualized care.

Plicní karcinom nadále svojí incidencí i mortalitou patří mezi první příčky zhoubných novotvarů v České republice. Správné rozhodnutí o léčbě je závislé na kvalitním stagingu, bioptickém vyšetření včetně prediktivních markerů, celkovém stavu pacienta a jeho komorbiditách a v neposlední řadě na přáních pacienta. Vzhledem k rozvíjejícím se léčebným možnostem je klíčové rozhodování ve zkušeném multidisciplinárním týmu (MDT), kdy v guidelines uvedená léčebná schémata jsou pro něj cenným vodítkem, ale ne vždy dogmatem. U malobuněčného plicního karcinomu (SCLC) lze u časných stadií zvažovat chirurgickou resekci doplněnou o chemoterapii. U lokálně pokročilého SCLC léčba spočívá s chemoradioterapii doplněné o následnou imunoterapii. Pokročilá stadia SCLC jsou pak obvykle léčená chemoimunoterapií. U nemalobuněčného plicního karcinomu (NSCLC) je nejčasnější stadium tumoru určeno k resekci. U vyšších operabilních stadií NSCLC (operabilitu posuzuje MDT) bez cílitelných mutací se obvykle volí neoadjuvantní chemoimunoterapie s následnou resekcí a případnou adjuvantní imunoterapií. U lokálně pokročilého inoperabilního NSCLC je metodou volby chemoradioterapie, případně doplněná (dle prediktivních markerů) o další léčbu. U metastazujícího onemocnění obvykle vybíráme z cílené léčby, imunoterapie či chemoimunoterapie. Léčebný tým by neměl opomíjet ani možnosti lokální léčby, tak i léčby podpůrné. V neposlední řadě musí mít léčebný tým dostatek zkušeností s léčbou navozených nežádoucích účinků.
Lung cancer continues to rank among the top malignant neoplasms in the Czech Republic in terms of incidence and mortality. The correct decision about treatment depends on high-quality staging, biopsy examination including predictive markers, the patient's overall condition and comorbidities, and last but not least, the patient's wishes. Given the evolving treatment options, decision-making is key in an experienced multidisciplinary team (MDT), where the treatment schemes listed in the guidelines are a valuable guide, but not always dogma. In small cell lung cancer (SCLC), surgical resection plus chemotherapy may be considered in early stages. In locally advanced SCLC, treatment consists of chemoradiotherapy plus subsequent immunotherapy. Advanced stages of SCLC are usually treated with chemoimmunotherapy. In non-small cell lung cancer (NSCLC), the earliest stage of the tumour is intended for resection. For higher operable stages of NSCLC (operability is assessed by MDT) without targetable mutations, neoadjuvant chemoimmunotherapy is usually chosen with subsequent resection and possible adjuvant immunotherapy. For locally advanced inoperable NSCLC, the method of choice is chemoradiotherapy, possibly supplemented (according to predictive markers) with other treatments. For metastatic disease, we usually choose from targeted therapy, immunotherapy or chemoimmunotherapy. The treatment team should not neglect either local treatment options or supportive treatment. Finally, the treatment team must have sufficient experience with treatment-induced adverse effects.

This study aimed to develop a vancomycin population pharmacokinetic model in obese adult patients treated with intermittent haemodialysis and propose a model-based loading dose strategy ensuring attainment of newly recommended AUC-based PK/PD target. Retrospective cross-sectional analysis was performed among obese haemodialysis dependent adult patients treated with intravenous vancomycin. A pharmacokinetic population model was developed using a nonlinear mixed-effects modelling approach and Monte Carlo simulations were used to identify the optimal loading dose for PK/PD target attainment during the first 48 h of treatment. Therapeutic drug monitoring data from 27 patients with a BMI of 30.2-52.9 kg/m2 were analysed. Among all tested variables, only LBM as a covariate of vancomycin Vd significantly improved the model, while vancomycin CL did not correlate with any of the tested variables. The median (IQR) value from the conditional mean of individual estimates of Vd and CL was 68.4 (56.6-84.2) L and 0.86 (0.79-0.90) L/h, respectively. To ensure optimal vancomycin exposure during the first 48 h of therapy, the vancomycin loading dose of 1500, 1750, 2000, 2250, 2500 and 2750 mg should be administered to obese patients with a lean body mass of ˂50, 50-60, 60-70, 70-80, 80-85 and >85 kg, respectively.
