V rámci mezinárodního projektu EUDAP-2 je realizován model primární prevence užívání návykových látek – program Unplugged, jenž je pilotně ověřován v podmínkách českých základních škol. Specifikem tohoto programu je jeho realizace školními metodiky prevence (ŠMP) přímo v rámci výuky – během dvanácti vyučovacích hodin v průběhu celého školního roku. Koordinátorem a realizátorem projektu je Centrum adiktologie 1. LF UK a VFN v Praze. CÍLE: Popsat proces implementace programu Unplugged, zjistit informace týkající se přípravy a realizace lekcí (jakým způsobem a za jakých podmínek byla implementace prováděna, v čem tkvěla úskalí a v čem naopak byla spatřována pozitiva programu), stejně jako zázemí školních metodiků prevence pro tuto činnost. Na základě zjištění navrhnout jeho inovace a změny. METODY: Metody kvalitativního výzkumu: sběr dat – semistrukturovaná interview; metody analýzy a interpretace – kódování, clustering, kategorizace, kontrast a srovnávání aj. VÝSLEDKY: Výsledky přinášejí impulz k úpravě témat s ohledem na specifika českých škol, zejména po terminologické stránce. Výsledky dále ukazují význam vstupního školení ŠMP před zahájením implementace programu stejně jako důležitost supervizních setkání vedených regionálními koordinátory a podpory managementu školy. Mezi hlavní pozitiva programu vnímaná ŠMP patří přínos pro třídní kolektiv jako takový a přínos pro metodikovu další práci. ZÁVĚR: Evaluace pilotního ověřování programu přinesla podněty pro přepracování manuálu pro učitele, metodiky, podle níž ŠMP programy připravují a realizují, stejně tak pro úpravu časové dotace lekcí a/nebo množství aktivit realizovaných v rámci jedné lekce, což je v souladu s výsledky již proběhlých zahraničních evaluačních studií.

The Unplugged programme, a model of non-specific primary prevention within the EUDAP-2 project, is implemented and evaluated in the setting of Czech elementary schools. The Unplugged programme is provided by the school prevention workers as part of the school curriculum (12 lessons per school year), which represents a specific trait of this programme. The main coordinator is the Centre for Addictology of the 1st Faculty of Medicine, Charles University in Prague. AIMS: The main aim of this text is to describe the process of implementation of the Unplugged programme and to obtain information related to the preparation and delivery of lessons, as well as data about the support which the school prevention workers receive for these activities. The findings will be used to suggest innovations and changes. METHODS: Qualitative research methods (semi-structured interviews), analysis and interpretation methods – coding, clustering, categorisation, contrast and comparison. SAMPLE: 16 participants in our research were recruited from among the school prevention workers from the Central Bohemia and South Moravia districts, using the purposive sampling method. RESULTS: The results will be used to initiate modification of the subject matter, including changes in terminology, to ensure that it reflects the specific needs of Czech schools. The results also emphasise the importance of introductory training for school prevention workers, as well as the importance of regular supervision and support from school management. The main benefits of the Unplugged programme as perceived by the school prevention workers included the promotion of team spirit among classmates and the enhancement of the workers’ future activities in this area. CONCLUSIONS: Evaluation of the programme’s pilot implementation brought suggestions for the revision of the Teacher’s Manual (a handbook according to which the school prevention workers prepare and deliver the lessons). Other suggestions include the modification of the time allocated to the lessons and/or the number of activities for one lesson. These findings correspond to foreign evaluation studies which have already been conducted.

Procesní model je při zvyšování kvality a zákaznické orientaci služeb organizací velmi efektivním pomocníkem. Procesní model spravovaný na platformě ARIS jasně popisuje realitu organizace ve všech pohledech (procesy a činnosti, organizační struktura, data, informační systémy, poskytované služby a výkony). Součástí takového popisu se mohou stát i požadavky standardů systémů řízení kvality jako je ISO či požadavky akreditačních programů. Úprava procesů organizace v souladu s požadavky těchto standardů je velmi efektivním způsobem, jak je implementovat a jak z nich realizovat maximální přínosy pro organizaci, neboť jsou brány v úvahu komplikované vazby a zdroje, které procesy a činnosti ovlivňují. Z procesního modelu je možné generovat směrnice nebo jej publikovat na intranetu, což velmi usnadňuje údržbu systému směrnic. Rovněž je snížena chybovost dokumentace, neboť vzájemné vazby a odkazy směrnic jsou ošetřeny v modelu a není nutné je pracně manuálně vyhledávat a aktualizovat. Vzhledem k architektuře ARIS u, který uchovává procesní model v databázi, je nezanedbatelná ani možnost analyzovat procesy a využívat informace o procesech jinými moduly platformy ARIS , jako je ARIS Process Performance Manager pro sledování výkonnosti procesů a ARIS Audit Manager pro nastavení interních kontrol, auditů a jejich řízení. Vytvořený procesní model je možné dále využívat při dalších úlohách projektech s orientací zejména na optimalizaci procesů, strukturování strategických cílů společnosti pomocí Balanced Scorecard, požadavky na informační systémy, řízení Enterprise Architecture apod.

Process model is effective helper in increasing service quality and customer orientation. Process model administrated on ARIS platform clearly describes reality in given organisation in all views (processes and functions, organizational structure, data, information systems, performance and services). Part of this description can be requirements on quality management systems such as ISO or accreditation standards. Design of company processes along with this requirements is very effective way how to implement the standards and how to realise maximum benefit out of them, because all complicated links and resources influencing processes are taken into account. There is possibility to generate internal procedures out of the process model or to publish it on the intranet, which is making easy administration of the procedures. Also the error rate is lowered because mutual links in procedures/processes are treated in model and it is not necessary to search for them manually. The ARIS architecture enables to store process model in database repository. This makes possible to analyse the processes and use the informations about them in other modules of ARIS platform like ARIS Process Performance Manager for monitoring of process performance and ARIS Audit Manager for setup of internal controlls, audits and their management. There is also possibility to use already created process model for another projects and tasks like process optimisation, structuring of stategic goals with Balanced Scorecards, requirements on information systems, management of Enterprise Archotecture and so on.

INTRODUCTION: Prevention science and quality standards of drug prevention in the EU and in the world have developed in recent decades. Progress has been made in quality development in Croatia, but the experience of the process of improving the field of drug prevention and the implementation of quality standards in Croatia has not been described in detail in the literature. This case study is an example of adaptation of the European QS and the emergence of other quality assurance components in the field of addiction prevention in the Republic of Croatia. The study describes the Croatian example of the implementation of QS from the perspective of the Service for Combating Drug Abuse (hereinafter SCDA) of the Croatian Institute of Public Health (hereinafter CIPH), and the context, processes, mechanisms, and activities that contributed to the implementation of QS in the Republic of Croatia. METHODS: Narrative review from database searches, literature review including grey literature, followed by a subsequent content analysis. RESULTS: The current study includes the background of the circumstances in which the implementation of QS occurred, as well as the activities that were performed. In addition, factors that could have contributed to the implementation of QS are presented. CONCLUSIONS: This Croatian case study suggests that different factors at a different level can influence QS implementation. The implementation of QS in the field of drug prevention is a long-term process influenced by various factors at the macro, mezzo, and micro levels. The study presents the shifts that have occurred as a result of the activities carried out and selected factors that could influence the implementation of QS at the micro, mezzo, and macro levels (or together). Reflection on the Croatian study on the implementation of QS may contribute and inspire/facilitate the implementation of QS and similar processes in other countries.

Autoři článku chtějí touto cestou informovat širokou odbornou veřejnost o nestandardní možností využití magnetické rezonance (MRI) v detekci a simulaci myoskeletálních dysfunkci. Tato metoda se pro zobrazení morfologie i funkce začala hojně využívat od roku 1990 díky rychlým zobrazovacím sekvencím a rozvoji tzv. funkční, phase-contrast-MRI nebo cine-MRI. V této době se začala morfologická data využívat také v oblasti biomedicínského modelování (kinematiká svalů a kloubů). Prezentované studie si kladou za cíl objektivizovat a modelovat situace, se kterými se setkáváme v běžné klinické praxi (konkrétně dysfunkce svalů v oblasti pánve či temporomandibulámího kloubu). Tyto stavy jsou výsledkem změn mechanických poměrů v měkkých tkáních. Experimentální soubor tvoří subjekty, u nichž klinické vyšetření neprokázalo přítomnost patologických zrněn v dané oblasti. Pro detekci tvarových změn jsme zvolili dva hlavní přístupy. V prvním případě sledujeme dva stavy pomocí MRI. Podstatou je intraindividuální srovnání stavu před a po nastalé změně (stav fyziologický a patologický). Druhým přístupem je využití MRI dat jako vstupních parametrů pro simulaci myoskeletálních dysfunkcí pomocí matematického modelování. Potřeba jednoduchého zpracování MRI snímků v trojdimenziálním prostoru nás vedla k vytvoření speciálních postupů v programu MANUAL MATCHING (SCAN VIEW), v 3D CAD systému EDS/UNIGRAPHICS NX2 a v neposlední řadě k využití programu TRANSFER. Tyto softwarové programy umožňují velice rychlou a precisní interpretaci snímané oblasti a interaktivní aplikace v 3D prostoru. Dílčím výstupem našich experimentů je nalezení optimální sekvence MRI, ve které by měkké struktury byly ve vzájemně vysokém kontrastu (TI TFE, SS Imm). V této sekvenci lze snímat po Imm, což se jeví jako optimální vzhledem k přesnosti rozlišení.

Authors of the paper want inform broad spectrum of health professionals, as well as general public audience, about non standard application of MRI in detection and simulation of musculoskeletal dysfunction. This method has been frequently used for imaging of the function, as well as the structure, since 1990 due to fast imaging sequences and also thanks to development of functional phase-contrast-MRI or cine ñ MRI. At this time there has been also use of these morphological data in the area of biomedical simulation (kinematics of the muscles and joints). Presented papers claim to objectify and simulate situations, which we can frequently meet at everyday routine clinical practice (especially muscle dysfunction of the pelvic area or TMJ). These statuses are the results of mechanical changes in soft tissues. Experimental set is composed of subjects, whose clinical examination did not proof presence of any pathological changes. For detection of structural changes we have chosen two principal approaches. In the first case we monitor the status before and after intervention using MRI. The substance of this process is intraindividual comparison of two states (before and after the change, status physiological and pathological). Second approach is the use of data gained from MRI examination, as enter data for simulation of mathematical models of musculoskeletal dysfunction. Need of simple evaluation of the MRI scans in 3D environment has lead us to creation of the Special programs MANUAL MATCHING (SCAN VIEW) in 3D CAD system EDS/UNIGRAPHICS NX2 as well as using the program TRANSFER. These software programs enable very fast and precise interpretation of scanned area and interactive applications in 3D dimension. Partial outcome of our experiments is to find optimal sequence of MRI where desired structures would be in required contrast (T1 TFE, SS 1mm). In this sequence you can scan layers after 1mm what is optimal for high definition.

Z důvodů vysoké prevalence užívání drog a rizikového chování u dětí a dospívajících a nedostatku programů indikované prevence, které se realizují ve školách v České republice, implementujeme do Českého prostředí efektivní program indikované prevence. Program Preventure vznikl v Kanadě v roce 2001, prošel testováním efektivity a vykazuje velmi slibné parametry a byl implementován v dalších zemích (např. Velká Británie, Nizozemí, Austrálie atd.). Tento program se skládá ze dvou částí, diagnostického screeningového dotazníku a intervenčních setkání face-to-face. Určen je pro cílovou skupinu dětí ve věku 12-15 let. CÍLE: Článek popisuje proces převodu a implementace indikované prevence do českého prostředí. Zároveň popisuje výsledky hodnocení tohoto procesu. kTÝSLEDKY: Metoda byla implementována dle kroků EMCDDA, konkrétně se jednalo o překlad a standardizaci diagnostického nástroje Substance Use Risk Profile Scale (SURPS). Dále překlad, adaptaci a grafickou úpravu metodických materiálů a podnětových sešitů pro žáky. Proběhl výcvik trenérů a školení facilitátorů metody Preventure. Realizovali jsme pilotní studii s programem, v návaznosti na to proběhly ohniskové skupiny a booster sessions po půl roce. Analýza zkušeností získaných z pilotního testování vedla k finální úpravě materiálů. ZÁVĚR: Program byl úspěšně adaptován na českou populaci a v pilotní fázi ním byly pozitivní zkušenosti. V současné době dochází k návrhu designu na realizaci randomizované kontrolované (RCT) studie efektivity Preventure. Metoda Preventure indikované prevence může po úspěšném ověření její efektivity zaplnit prázdné místo v preventivních aktivitách realizovaných na českých školách.

BACKGROUND: Despite the high prevalence of drug use and other forms of risk behaviour among children and adolescents, only a few indicated prevention programmes are delivered in Czech schools. Therefore, we decided to implement Preventure in the Czech setting. It is an evidence-based indicated prevention programme which was developed in Canada in 2001. The programme was tested for its effectiveness and shows very promising results. It is intended for children aged 12-15. AIMS: The manuscript describes the process of adapting and implementing this indicated prevention tool in the Czech setting, as well as presenting the results of the evaluation of the process. RESULTS: The method was implemented following the steps proposed by the EMCDDA. Specifically, it involved the translation and standardisation of the Substance Use Risk Profile Scale (SURPS). The methodological guidelines and student handbooks were translated, adapted, and modified in graphic terms. Preventure trainers and facilitators were trained. The programme was tested in a pilot study followed by focus groups and booster sessions six months later. Analysis of the experience obtained during the pilot testing was used to make the final adjustments of the materials. CONCLUSION: The positive outcomes of the pilot stage of the implementation of the programme suggested that its adaptation to the Czech population was successful. A randomised controlled trial (RCT) is currently being designed to test the effectiveness of Preventure .If its effectiveness in the Czech setting is demonstrated, this method of indicated prevention may fill a gap in the preventive activities in Czech schools.

Objetivo: El proceso de integración de sistemas de información en salud es complejo, especialmente cuando se deben cumplir requisitos de interoperabilidad en los niveles semántico y de la lógica del negocio. Para tener éxito en un proyecto de integración de estas características, es fundamental el uso de una metodología de desarrollo unificada que permita analizar de forma separada diferentes aspectos de la arquitectura del sistema como por ejemplo los procesos de la organización, la información a ser procesada, la distribución de componentes, así como aspectos específicos de las tecnologías a emplear. Este artículo contribuye con un análisis detallado y una demostración de cómo el estándar HL7 puede ser usado para soportar el desarrollo de proyectos de integración de sistemas de información en salud. Métodos: La metodología de desarrollo unificada a utilizar es el Marco Integral de Desarrollo para Sistemas de Información en salud (HIS-DF). HIS-DF es una metodología basada en diferentes estándares, genérica, personalizable y escalable; que soporta el diseño de sistemas de información semánticamente interoperables. Basados en HIS-DF, se analizan tres modelos genéricos para la integración de sistemas de información en salud. Resultados:Los modelos arquitectónicos para integración de sistemas analizados son: Interfaz Punto a Punto, Servidor de Mensajes y Mediación. Los modelos de Interfaz Punto a Punto y Servidor de Mensajes son completamente soportados por sistemas tradicionales de mensajería HL7 versión 2 y versión 3. El estándar HL7 versión 3, combinado con tecnologías orientadas a servicios y la metodología HIS-DF, hacen posible el modelo de Mediación. Los diferentes escenarios de integración son ejemplificados mediante el desarrollo de un sistema de vigilancia de la salud pública basada en la tecnología de Enterprise Java Beans. Conclusión: La selección de la arquitectura de integración es una decisión fundamental de cualquier proyecto de desarrollo de software. HIS-DF provee un enfoque metodológico único para guiar el desarrollo de proyectos de integración de sistemas de información en salud. El modelo de Mediación –ofrecido por la metodología HIS-DF apoyada en artefactos del estándar HL7 versión 3– es el modelo más prometedor de los tres analizados, pues promueve el desarrollo de sistemas de información en salud abiertos, reutilizables, flexibles, semánticamente interoperables, independientes de la plataforma, orientados a servicios y basados en estándares.

Objective: Information systems integration is hard, especially when semantic and business process interoperability requirements need to be met. To succeed, a unified methodology, approaching different aspects of systems architecture such as business, information, computational, engineering and technology viewpoints, has to be considered. The paper contributes with an analysis and demonstration on how the HL7 standard set can support health information systems integration. Methods: Based on the Health Information Systems Development Framework (HIS-DF), common architectural models for HIS integration are analyzed. The framework is a standard-based, consistent, comprehensive, customizable, scalable methodology that supports the design of semantically interoperable health information systems and components. Results: Three main architectural models for system integration are analyzed: the point to point interface, the messages server and the mediator models. Point to point interface and messages server models are completely supported by traditional HL7 version 2 and version 3 messaging. The HL7 v3 standard specification, combined with service-oriented, model-driven approaches provided by HIS-DF, makes the mediator model possible. The different integration scenarios are illustrated by describing a proof-of-concept implementation of an integrated public health surveillance system based on Enterprise Java Beans technology. Conclusion: Selecting the appropriate integration architecture is a fundamental issue of any software development project. HIS-DF provides a unique methodological approach guiding the development of healthcare integration projects. The mediator model – offered by the HIS-DF and supported in HL7 v3 artifacts – is the more promising one promoting the development of open, reusable, flexible, semantically interoperable, platform-independent, service-oriented and standard-based health information systems.
