BACKGROUND: Currently, used oral antiplatelet drugs are both limited and associated with the risk of treatment failure/resistance. Research in this area is hence highly desired. A series of xanthene-3-ones derivatives, we had synthesized, showed us that these derivatives had antiplatelet activity. As far as we know, no research on the effects of xanthen-3-ones in this area has been done. OBJECTIVE: The aim was to study the antiplatelet potential of a series of synthesised 9-phenylxanthene- 3-ones and to find the ideal structural feature(s) for antiplatelet potential and determine the mechanism of action. METHODS: The compounds were synthesized from 1,2,4-triacetoxybenzene and various benzaldehydes. The reaction proceeded smoothly under acidic alcoholic conditions, furnishing the desired products in good yields. The compounds were first screened in whole human blood where platelet aggregation was induced by arachidonic acid. Further analysis was targeted at search of the mechanism of action. RESULTS: Initial screening showed that a majority of the synthesized derivatives had substantial antiplatelet potential. None of the compounds were able to block cyclooxygenase 1 or thromboxane synthase. The mechanism appeared to be based on antagonism of thromboxane effects. The most potent compound 9-(4'-dimethylaminophenyl)-2,6,7-trihydroxy-xanthene-3-one had better potential to block collagen induced platelet aggregation than clinically used acetylsalicylic acid. CONCLUSION: The last mentioned derivative is promising for further in vivo testing.

AIMS: Platelet function testing is often affected by the existence of different pre-analytical variables that can cause platelet activation. The aim of this study was to assess the effect of such variables that are present when samples are taken (different anticoagulants, incubation temperature, and storage conditions) to select those which enable to reach optimal range of measured plasma concentrations of the two stable thromboxane A₂ metabolites, that is, thromboxane B₂ (TxB₂) and 11-dehydrothromboxane B₂ (11-dTxB₂). MATERIALS AND METHODS: For the purpose of this study, whole blood samples obtained from 20 volunteers were screened for TxB₂ and 11-dTxB₂ concentrations using commercial EIA kits (Cayman Chemicals™; Neogen™) in relation to the effect of different anticoagulants, using different incubation temperatures and storage conditions. RESULTS: Trisodium citrate has been shown not to be affecting the TxB₂ and 11-dTxB₂ concentrations compared with the values measured in the serum. Incubation of the samples for 1 h at 37 °C and freezing at -20 °C or -80 °C give the most suitable concentration range of both thromboxanes in the used EIA measurement. CONCLUSION: This study describes the setup of such pre-analytical phase conditions that enable the screening of platelet function in terms of the plasma concentrations of TxB₂ and 11-dTxB₂ in selected EIA measurement.

There are only few areas of cardiology that have witnessed such dramatic innovations as that occurring in the treatment and prophylaxis of thrombotic events. Antithrombotic (i.e., antiplatelet and anticoagulation) therapy plays a pivotal role in the prophylaxis of the pandemic of cardiovascular disease. Given the host of triggers activating primary hemostasis, various therapeutic strategies are currently available. The current approach, monotherapy or dual therapy or, possibly, combination therapy with antiplatelet and anticoagulant agents is selected based on the risk of a thrombotic event, dominant disease, and the risk of bleeding.The main problem associated with the current therapeutic strategy was not an insufficient effect, but major inter-individual variability of effect resulting in therapy failure or an unacceptable risk of bleeding documented in a non-negligible proportion of patients. Hence there is a drive for devising new antiplatelet strategies and innovation in (already) established classes of drugs.Milestones in the evolution of antiplatelet therapy included the advent of new, more effective and/or safer antiplatelet agents inhibiting platelet activation. The new irreversible P2Y12 receptor antagonist prasurgel and reversible P2Y12 receptor antagonist ticagrelor have been approved for clinical use. There has also been major progress in the development of thrombin protease-activated receptor 1 (PAR-1) antagonists (vorapaxar, atopaxar) or serotonin receptor blockers (sarpogrelat). Another promising therapeutic strategy is targeted at platelet stabilization through increased cyclic adenosine monophosphate (cAMP) activity by platelet phosphodiesterase-3 inhibition (cilostazol). Further, new insights into the bioavailability of acetylsalicylic acid under specific conditions have been reported regarding the class of agents inhibiting thromboxane A2-mediated activation.

Strategie léčby a profylaxe aterotrombotických příhod je postavena na třech pilířích – na zastavení progrese aterogeneze (zejména úpravou rizikových faktorů, tj. životosprávou a farmakoterapií), na zabránění trombotických komplikací (tj. na antitrombotické léčbě) a na úpravě případných poruch cirkulace vedoucích k ischémii (tj. v revaskularizaci či podávání vazoaktivních léčiv). U ischemické choroby dolních končetin je antitrombotická léčba založena na protidestičkové léčbě. Pouze při vysokém riziku trombózy (např. po uzávěru cévní spojky) či po proběhlé embolizaci do periferní artérie spočívá v inhibici primární i sekundární hemostázy, tj. na kombinaci protidestičkových léků s antikoagulancii. V profylaxi trombotických příhod u nemocného s ischemickou chorobou dolních končetin či, obecně řečeno, při aterosklerotickém postižení periferních částí systémové cirkulace volíme k úpravě funkce hyperaktivovaných trombocytů u asymptomatických nemocných podání kyseliny acetylsalicylové v dávce 75–100 mg (stupeň evidence 2B). U symptomatických nemocných či u nemocných po revaskularizaci je vedle kyseliny acetylsalicylové alternativou klopidogrel, pro oba léky je doložena účinnost na nejvyšší úrovni (evidence 1A). V porovnání s kyselinou acetylsalicylovou má klopidogrel výraznější efekt na pokles trombotických komplikací při srovnatelném riziku krvácivých komplikací (evidence 1B). Výhledově je možno očekávat zavedení nových léčiv, zejména cilostazolu s vazodilatačním a protidestičkovým účinkem či potvrzení efektu nových blokátorů ADP receptorů – tikagreloru a prasugrelu – též v indikaci léčby ICHDK.
The strategy of treatment and prophylaxis of atherothrombotic accidents is based on three „pillars“ - stopping the progression atherogenesis (mostly by managing risk factors, i. e. lifestyle and pharmacotherapy, preventing thrombotic complications (antithrombotic treatment) and managing any applicable circulating disorders causing ischemia (i. e. revascularisation or administering vasoactive drugs). When preventing aterothrombotic complications in lower limbs, the therapy is based on antiplatelet treatment. Only in cases of very high risk of thrombosis (for example after blockage of a vascular connection) or after a peripheral artery embolism, the therapy consists of inhibition of both primary and secondary haemostasis, i. e. a combination of antiplatelet drugs with anticoagulants. In prophylaxis of thrombotic accidents in patients suffering from lower limb ischemia, or generally in cases of atherosclerotic damage to peripheral parts of the circulatory system, acetylsalicylic acid is used, in doses of 75–100 mg (equivalency grade 2B). In symptomatic patients or patients after revascularisation, clopidogrel is an alternative to acetylsalicylic acid; both the drugs have their efficacy verified to the highest degree (evidence 1A). In comparison to acetylsalicylic acid, clopidogrel has a more significant effect on decrease of thrombotic complications, with a comparable risk of bleeding-related complications (evidence 1B). In near future, it is possible to expect introduction of new drugs, especially cilostasol (not yet available in CR) with vasodilatory and antiplatelet effects and confirmation of efficacy of new ADP receptor blockers – ticagrelor and prasugrel – in this particular indication.

Koxiby byly vyvinuty jako alternativa podávání konvenčních nesteroidních antirevmatik za účelemsnížení rizika vzniku gastrointestinálních komplikací. V souvislosti se studií VIGOR se začaladiskutovat otázka kardiovaskulární bezpečnosti podávání koxibů, neboť pacienti, kteří dostávalive studii rofekoxib, měli vyšší výskyt nefatálního infarktu myokardu v porovnání s naproxenem.Výsledky dalších studií ověřujících bezpečnost podávání koxibů jsou kontroverzní, jedná se všakvětšinou o retrospektivní observační populační studie a negativní vliv na kardiovaskulární aparátv nich dosud nebyl bezpečně prokázán. Mechanismus, který by mohl být důvodem zvýšenéhovýskytu kardiovaskulárních příhod, je nejasný, uvažuje se o změně poměru prostacyklinu (PGI2)a tromboxanu (TXA2) v důsledku podávání inhibitorů cyklooxygenázy-2. Je také možné, že se vlivna kardiovaskulární aparát liší mezi jednotlivými představiteli koxibů. V poslední době se objevujízprávy, že by např. celekoxib mohl mít dokonce kardioprotektivní efekt, neboť zlepšuje endoteliálnídysfunkci u pacientů s ischemickou nemocí srdeční a s hypertenzí. Z uvedeného přehledu studiíověřujících renální bezpečnost podávání koxibů vyplývá, že inhibitory cyklooxygenázy-2 nepřinášejívětší renální bezpečnost v porovnání s konvenčními NSA, a to s ohledem na ovlivnění ledvinnýchfunkcí, retenci sodíku i vývoj hypertenze.
