OBJECTIVE: "Krokodil" is the street name for an impure homemade drug mixture used as a cheap substitute for heroin, containing desomorphine as the main opioid. Abscesses, gangrene, thrombophlebitis, limb ulceration and amputations, jaw osteonecrosis, skin discoloration, ulcers, skin infections, and bleeding are some of the typical reported signs in humans. This study aimed to understand the toxicity of krokodil using Wistar male rats as experimental model. METHODS: Animals were divided into seven groups and exposed subcutaneously to NaCl 0.9% (control), krokodil mixture free of psychotropic substances (blank krokodil), pharmaceutical grade desomorphine 1 mg/kg, and four different concentrations of krokodil (containing 0.125, 0.25, 0.5, and 1 mg/kg of desomorphine) synthesized accordingly to a "domestic" protocol followed by people who inject krokodil (PWIK). Daily injections for five consecutive days were performed, and animals were sacrificed 24 hr after the last administration. Biochemical and histological analysis were carried out. RESULTS: It was shown that the continuous use of krokodil may cause injury at the injection area, with formation of necrotic zones. The biochemical results evidenced alterations on cardiac and renal biomarkers of toxicity, namely, creatine kinase, creatine kinase-MB, and uric acid. Significant alteration in levels of reduced and oxidized glutathione on kidney and heart suggested that oxidative stress may be involved in krokodil-mediated toxicity. Cardiac congestion was the most relevant finding of continuous krokodil administration. CONCLUSIONS: These findings contribute notably to comprehension of the local and systemic toxicological impact of this complex drug mixture on major organs and will hopefully be useful for the development of appropriate treatment strategies towards the human toxicological effects of krokodil.

"Krokodil" is the street name for a drug, which has been attracting media and researchers attention due to its increasing spread and extreme toxicity. "Krokodil" is a homemade injectable mixture being used as a cheap substitute for heroin. Its use begun in Russia and Ukraine, but it is being spread throughout other countries. The starting materials for "krokodil" synthesis are tablets containing codeine, caustic soda, gasoline, hydrochloric acid, iodine from disinfectants and red phosphorus from matchboxes, all of which are easily available in a retail market or drugstores. The resulting product is a light brown liquid that is injected without previous purification. Herein, we aimed to understand the chemistry behind "krokodil" synthesis by mimicking the steps followed by people who use this drug. The successful synthesis was assessed by the presence of desomorphine and other two morphinans. An analytical gas chromatography-electron impact/mass spectrometry (GC-EI/MS) methodology for quantification of desomorphine and codeine was also developed and validated. The methodologies presented herein provide a representative synthesis of "krokodil" street samples and the application of an effective analytical methodology for desomorphine quantification, which was the major morphinan found. Further studies are required in order to find other hypothetical by-products in "krokodil" since these may help to explain signs and symptoms presented by abusers.

Its use begun in Russia and Ukraine and nowadays is being spread over several other countries. Desomorphine is the semi-synthetic opioid claimed to be the main component of krokodil and considered to be responsible for its psychoactive characteristics. The starting materials for desomorphine synthesis are codeine tablets, alkali solutions, organic solvent, acidified water, iodine and red phosphorus, all of which are easily available in retail outlets, such as supermarkets, drugstores, etc. The resulting product is a light brown liquid that is called krokodil. People who inject krokodil present a great variety of serious signs and symptoms, including thrombophlebitis, ulcerations, gangrene, and necrosis, quickly evolving to limb amputation and death. These effects are thought to result from the toxic components produced as byproducts during the homemade drug synthesis. In this work, we reviewed several aspects of krokodil use, including its epidemiology, pharmacology and the chemical properties of the main active ingredient (desomorphine). To enhance our understanding of the clinical and toxic effects and to support the implementation of harm reduction measures, we also describe the "bathtub chemistry" of krokodil and the content of the final solution.

Účel štúdie: Cieľom bolo stanoviť trendy v spotrebe silných a slabých opioidných analgetík v priebehu prvej dekády 21.storočia. Metódy: Údaje sme získali z databázy Štátneho ústavu pre kontrolu liečiv SR. Hodnotenie spotreby sme realizovali pomocou kritéria denných definovaných dávok na tisíc obyvateľov a jeden deň. Získané výsledky sme porovnali s údajmi z českého Státního ústavu na kontrolu léčiv a na porovnanie so zahraničím sme použili údaje z nórskych štatistík. Výsledky: Slovensko má násobne vyššiu spotrebu silných opioidov v porovnaní s Českou republikou. Pri hodnotení spotreby najmä silných opioidných analgetík je vidno výraznú dominanciu Nórska. Spotreba pri všetkých sledovaných liečivách má stúpajúcu tendenciu a jedinou výnimkou je relatívne ustálená spotreba buprenorfínu na Slovensku a pokles spotreby morfínu vo všetkých sledovaných krajinách. Závery: Ostáva nám len vyjadriť nádej pokračovania nastúpených trendov rastu spotreby opioidných analgetík. Lebo nemôžme tvrdiť, že by u nás bolo menej bolesti.
Objective: To assess trends in consumption of strong and weak opioid analgesics during the first decade of the 21st century. Methods: Data were collected from the State Institute for Drug Control in Slovakia. Evaluation was done using the methodology of defined daily doses per 1000 inhabitants per day. The data obtained were compared with those from the Czech State Institute for Drug Control and also comparison with statistics from Norway was done. Resuts: Slovakia has several times higher consumption of strong opioids when compared with the Czech Republic. When evaluating the consumption of strong opioids in particular, Norway is clearly dominant. Consumption of all the opioids observed is increasing except a relatively stable consumption of buprenorphine in Slovakia and a decrease in morphine consumption in all the countries. Conclusions: We hope that these increasing trends will continue. Because we cannot say that there is less pain.

Levodropropizin je levotočivým izomerem dropropizinu, nekodeinového antitusika s periferním mechanismem působení. Vykazuje antitusické účinky, zejména periferního typu na tracheobronchiální úrovni, doprovázené účinky antialergickými a antibronchospastickými. Účinnost levodropropizinu je srovnatelná s účinností ostatních antitusik jak kodeinového (dextrometorfan, dihydrokodein), tak i nekodeinového typu (dropropizin), nebo je lepší. Jeho podávání přitom bývá provázeno nižším výskytem nežádoucích účinků včetně ospalosti. Účinnost a snášenlivost levodropropizinu byla prověřena na dospělých pacientech a více než 1 000 dětských pacientech.

Cílem práce bylo posouzení dihydrokodeinu v systému s rízeným uvolnováním v lécbe chronické bolesti u starších nemocných. Z multicentrického prospektivního klinického sledování zamereného na efektivitu, bezpecnost a celkovou užitecnost dihydrokodeinu v lécbe chronické bolesti byly analyzovány výsledky docílené v skupine pacientu ve veku 65 let a více. Dihydrokodein byl podáván v dávce 60 mg 2 x denne, v prípade potreby zvýšené do 120 mg 2x denne. Bolest byla hodnocena vizuální analogovou škálou (VAS) a verbální škálou. Denne bylo sledováno trvání bolesti, doba spánku a nežádoucí úcinky lécby. Výkonnostní stav pacientu byl posouzen dle kriterií WHO v 1., 8. a 14. den sledování. Ke statistickému zhodnocení výsledku byl použit Wilcoxonuv znaménkový poradový test. Zjištené bylo vysoce prukazné snížení bolesti (p < 0,001) pri porovnání VAS a verbálních hodnot bolesti pred zahájením lécby a pri záverecném hodnocení. U 3 pacientu bylo potrebné zvýšit dávku dihydrokodeinu nad 2 x 60 mg denne. Výkonnostní stav se zlepšil u 12 z 28 pacientu (p < 0,01). Pouze ve 2 prípadech byla potrebná zmena analgetika v dusledku nežádoucích úcinku lécby (vomitus a obstipace). Pri záverecném zhodnocení byl dihydrokodein posouzen jako užitecný anebo velice užitecný u 25 (89 %) pacientu.
The aim of this study was to evaluate controlled-release dihydrocodeine in the treatment of chronic pain in elderly patients. Data from prospective, multicentric evaluation of the efficacy, safety and overall usefulness of dihydrocodeine for chronic pain were analyzed in a group of 28 patients aged 65 years or over. Controlled-release dihydrocodeine was administered at a dose of 60 mg b.i.d. (raised to 120 mg b.i.d. as needed). Pain-rating scales (verbal and VAS) were used for pain assessment. Duration of pain, sleep and adverse effects of treatment were evaluated daily, patients WHO performance status on days 1, 8, and 14. Wilcoxon’s sign-rank test was used for statistical analyses of results. A highly significant (p<0.001) decrease in pain intensity was confirmed by comparing VAS and verbal pain intensity levels at baseline (before the patients entered the study) and by the time of final evaluation. Three patients required to have the dose of dihydrocodeine increased to over 60 mg b.i.d. Performance status improved in 12 of the 28 patients (p<0.01). In only 2 patients was it needed to change analgesics because of side effects (vomiting and constipation). In final evaluation, dihydrocodeine was considered to be very useful or useful in 25 (89%) patients.
