The authors describe the possibilities of radiotherapy of prostate cancer and focus on certain controversies associated withproton therapy in this indication. They bring up to date published data on the results and risks associated with proton therapyin prostate irradiation. The work also contains presented case studies of patients with proton-related complications who havebeen or are being treated for urology.
Autoři popisují možnosti radioterapie karcinomu prostaty a soustřeďují se na určité kontroverze spojené s protonovou léčbou v této indikaci. Přinášejí dosud publikovaná data týkající se výsledků a rizik spojených s protonovou terapií při ozáření prostaty. Práce obsahuje také prezentované kazuistiky pacientů s komplikacemi po protonové terapii, kteří byli nebo jsou léčeni na urologii.

V článku je podán ucelený pohled na problematiku karcinomu mléčné žlázy se zaměřením na jeho léčbu s dělením do kapitol podle přítomnosti vzdálené diseminace, léčbě karcinomu prsu u těhotných žen, karcinomu prsu u mužů a zmíněn je přístup k jedincům s BRCA pozitivitou. Snahou bylo podat spíše zobecňující pohled na tuto obsáhlou problematiku.
In this article there is a complete look at breast cancer focused on its treatment. It is divided into chapters by the presence of distantmetastasis, the treatment of breast cancer in pregnant women and breast cancer in men. Approach to BRCA positive patients ismentioned too. The effort was to give generalised view on this comprehensive issue.

Primární kožní lymfomy patří mezi mimouzlinové nehodgkinské lymfomy, které jsou charakterizované přítomností maligních T nebo B lymfocytů v kůži. Většina primárních kožních lymfomů je zachycena v časných stadiích a má indolentní průběh s dobrou prognózou. Případy nemocí s agresivním chováním a nutností kombinované systémové a zevní léčby však nejsou výjimkou. Přehledový článek shrnuje diagnostiku a léčbu nejdůležitějších T- a B -buněčných kožních lymfomů s důrazem na dermatologickou manifestaci a na léčbu cílenou na kůži.
Primary cutaneous lymphomas are a rare type of extranodal non - Hodgkin lymphomas where malignant T-cells or B-cells infiltratethe skin. Most patients with primary cutaneous lymphomas are diagnosed in the early stage of the disease with usually indolentclinical course. However, cases with aggresive behaviour and unfavourable prognosis are not rare. This review summarizes diagnosisand treatment of the most important subtypes of cutaneous lymphomas with emphasis on dermatological manifestationsand skin directed therapy.

Mozkové metastázy představují důležitou a častou indikaci pro moderní radiochirurgii. Tato neinvazivní metoda nabízí dobrou lokální kontrolu s minimální toxicitou s možností opakování ozáření. CyberKnife je přístroj pro bezrámovou celotělovou radiochirurgii využívající obrazem řízenou robotickou technologii s lineárním akcelerátorem jako zdrojem fotonů. V naší prospektivní studii vyhodnocujeme velikost metastatických ložisek na MRI s využitím počítačově asistované volumetrie, přičemž srovnáváme prediktivní hodnotu objemových změn ozářených metastáz. Sledujeme efekt léčby v závislosti na histologickém typu nádoru, počtu mozkových metastáz, pohlaví a vstupním klinickém stavu pacientů. V našich podmínkách unikátní soubor pacientů léčených CyberKnife pro metastatické postižení mozku pak statisticky analyzujeme a výsledky srovnáváme s literárními daty pracovišť využívajících stejnou technologii i referenčními daty center rámové stereotaktické radiochirurgie (gama nůž).
Brain metastases are an important and frequently treated indication in modern radiosurgery. This non-invasive method offersgood local control with minimum toxicity and repeability of treatment. The CyberKnife is a device for frameless whole-bodyradiosurgery using image guided robotic technology and linear accelerator as a photon source. In our prospective study wemeasure the volume of brain metastases on MRI with use of computer-assisted volumetry and we evaluate the predictive valueof these volumetric changes. We track the effect of therapy according to histological type of tumor, number of brain metastases,sex and initial performance status of patients. The group of patients with CyberKnife treateted brain metastases, that is uniquein our conditions, is then statistically analyzed and results are compared to peer-reviewed articles of institutions using the sametechnology and reference data from frame-based (Gamma Knife) radiosurgery centers are matched.

Anti-HER2 léčba pertuzumabem a trastuzumabem u časného karcinomu prsu snižuje riziko relapsu onemocnění o 1 % proti samotnému trastuzumabu při mediánu sledování 45,4 měsíce.
Anti-HER2 treatment with pertuzumab and trastuzumab in early breast cancer reduces the risk of relapse by 1 % against trastuzumab alone at median follow‑up of 45.4 months.
